Medicare might have saved billions if not for the competition from "skinny label" generics.

Medicare Part D saved almost $15 billion between 2015 and 2021 thanks to competition from "skinny label" generic drugs, according to an analysis of 15 name-brand medications and their equivalents. Generic drugs are allowed by thin labeling.
