Flu Survey questions

Dear Doctor - Participate in the Survey on flu "Each year, millions of people in the United States get the flu. The Centers for Disease Control and Prevention (CDC) conducts a yearly survey to find out how many people have the flu and how severe it is.The CDC asks people of all ages about their health during a randomly selected two-week period. They ask questions such as whether they have had a fever in the last week, whether they have felt tired or had muscle aches, and whether they have had a cough or sore throat. They also ask about any doctor visits, hospitalizations, or deaths due to the flu.The results of the survey help the CDC track the spread of the flu and determine how severe it is. The results are used to plan for next year’s flu season and to develop recommendations for preventing and treating the flu. Participate now."
