Chronic cough, defined as a cough lasting more than eight weeks, is a prevalent and debilitating condition that can significantly impair quality of life. It poses a complex diagnostic and therapeutic challenge due to its multifactorial etiology and the necessity for a comprehensive and systematic approach.
A thorough patient history and physical examination are crucial in the initial evaluation. Key considerations include the duration, severity, triggers, and associated symptoms of the cough. Diagnostic tests such as chest radiography, spirometry, and bronchoscopy may be necessary depending on the clinical suspicion. A trial of empirical therapy can also provide diagnostic clues.
The most common causes of chronic cough in adults are upper airway cough syndrome (UACS), asthma, gastroesophageal reflux disease (GERD), and non-asthmatic eosinophilic bronchitis (NAEB). However, it is important to note that these conditions often coexist, and multiple diagnoses may be present in a single patient.
Treatment should be etiology-specific and may involve lifestyle modifications, pharmacologic therapy, or surgical interventions. For example, proton pump inhibitors may be beneficial in GERD-related cough, while inhaled corticosteroids are the treatment of choice for asthma and NAEB. Refractory cases may require referral to a pulmonologist or otolaryngologist for further evaluation and management.
Chronic cough is a complex condition that requires a comprehensive and systematic approach to diagnosis and management. By understanding the common etiologies and implementing appropriate treatment strategies, healthcare professionals can significantly improve patient outcomes and quality of life.
