Fatigue can be both a symptom and a disease entity, often blurring the lines of diagnosis. It is characterized by a subjective lack of physical and/or mental energy perceived by the individual or caregiver to interfere with usual and desired activities. It is crucial to differentiate fatigue from related constructs like weakness, dyspnea, or depression to ensure appropriate management.
Assessing fatigue involves a detailed patient history, physical examination, and targeted investigations. The history should focus on the onset, duration, pattern, and associated symptoms. Physical examination can reveal signs of systemic diseases or neurological deficits. Investigations should be guided by clinical suspicion, ranging from basic blood tests to more specialized studies.
Management of fatigue is primarily symptom-based and tailored according to the underlying cause. Non-pharmacological interventions are the mainstay of treatment, including cognitive-behavioral therapy, graded exercise therapy, and lifestyle modifications. Pharmacological interventions can be considered in certain cases, such as modafinil for narcolepsy-associated fatigue.
Understanding and managing fatigue requires a comprehensive, patient-centered approach. It is essential to consider fatigue as a multifaceted symptom, requiring thorough assessment and a tailored management plan. By doing so, healthcare professionals can significantly improve the quality of life of patients suffering from fatigue, ultimately enhancing their overall healthcare experience.
