Interstitial lung diseases (ILDs) represent a heterogeneous group of more than 200 pulmonary disorders classified by inflammation and fibrosis of the interstitium. These diseases have a significant impact on patient morbidity and mortality, requiring an in-depth understanding for effective management.
ILDs can be idiopathic or secondary to systemic diseases, environmental exposures, or drug toxicity. The most common form is idiopathic pulmonary fibrosis (IPF), characterized by a progressive decline in lung function. Other forms include nonspecific interstitial pneumonia (NSIP), hypersensitivity pneumonitis (HP), and ILDs associated with connective tissue diseases.
Patients typically present with progressive dyspnea and non-productive cough. Diagnostic evaluation involves a careful history, physical examination, pulmonary function tests, high-resolution computed tomography (HRCT), and sometimes, lung biopsy. HRCT is particularly valuable in providing detailed images of the lung parenchyma, aiding in diagnosis and disease classification.
Management strategies depend on the underlying cause and disease severity. For IPF, antifibrotic agents such as pirfenidone and nintedanib have shown to slow disease progression. In other ILDs, immunosuppressive therapies may be beneficial. Lung transplantation remains the ultimate treatment option for selected patients. Prognosis varies widely, with IPF carrying a poor prognosis with a median survival of 2-3 years post-diagnosis.
ILDs are complex diseases requiring a comprehensive approach for diagnosis and management. A thorough understanding of the various forms, their clinical presentations, and management strategies is crucial for healthcare professionals. Ongoing research into the pathogenesis and treatment of these diseases promises to offer new insights and therapeutic options in the future.
