Vertigo, a common clinical symptom, is often a diagnostic challenge due to its multifaceted nature. It is crucial for healthcare professionals to understand the underlying pathophysiology and adopt a systematic approach to diagnose and manage this condition effectively.
Vertigo is a subjective sensation of movement, usually rotational, experienced by patients. It is primarily a symptom of vestibular dysfunction, which may arise from peripheral or central causes. Peripheral causes include benign paroxysmal positional vertigo (BPPV), Meniere's disease, and vestibular neuritis while central causes encompass cerebrovascular disorders and migraines.
A comprehensive history and physical examination are paramount in diagnosing vertigo. The onset, duration, triggers, and associated symptoms can provide valuable clues. Physical examination should include a thorough neurological and otoscopic examination. Special tests like the Dix-Hallpike maneuver or the head impulse test can aid in diagnosis.
When history and physical examination are inconclusive, additional diagnostic tests may be required. These include vestibular function tests, audiometry, and imaging studies like MRI. It is important to choose the most appropriate test based on the patient's presentation to avoid unnecessary investigations.
Management of vertigo is multifaceted and includes symptomatic treatment, vestibular rehabilitation, and treating the underlying cause. Medications like antihistamines, benzodiazepines, and calcium channel blockers are commonly used. In cases like BPPV, canalith repositioning maneuvers can be effective. For central causes, a multidisciplinary approach involving neurologists and physiotherapists may be necessary.
Vertigo, while common, is a complex clinical symptom requiring a comprehensive clinical approach for accurate diagnosis and management. A thorough understanding of its pathophysiology, coupled with a systematic approach to assessment and management, can help healthcare professionals provide effective care to patients suffering from vertigo.
