Unexplained weight loss (UWL), defined as a loss of 5% or more of total body weight within a six to twelve month period without an apparent cause, poses a diagnostic challenge for clinicians. It may be an early sign of a serious underlying condition requiring prompt identification and treatment.
UWL can be attributed to a myriad of causes ranging from malignancies, endocrine disorders, gastrointestinal diseases, to psychiatric illnesses. However, in approximately 15-25% of cases, the cause remains elusive despite extensive evaluation. This necessitates a comprehensive, systematic approach to effectively manage these patients.
A thorough history and physical examination are the cornerstones of the diagnostic approach. They can provide valuable clues about potential etiologies. Laboratory testing should be guided by the clinical suspicion. Empirical trials of treatment may be considered if initial investigations are non-revealing.
Imaging studies have a pivotal role in the evaluation of UWL. They can detect occult malignancies, chronic infections, or other systemic diseases. The choice of imaging modality should be individualized based on the patient's history and physical findings.
Collaboration between various specialties is crucial in managing patients with UWL. A multidisciplinary approach can expedite the diagnostic process, improve patient care, and potentially reduce healthcare costs.
Unexplained weight loss is a complex clinical problem that requires a comprehensive and systematic approach. A thorough history, physical examination, and judicious use of laboratory and imaging studies are essential. Collaboration among various specialties can optimize patient outcomes. While the cause may remain elusive in some cases, a meticulous approach can help in the early detection and management of serious underlying conditions.
