This review synthesizes current literature, clinical guidelines, and expert consensus to provide an in-depth framework for physicians managing patients following significant metabolic body composition changes, emphasizing risk identification, diagnostic approaches, individualized therapy, and monitoring protocols to ensure durable metabolic health improvements and mitigate adverse sequelae.

Introduction

The prevalence of metabolic disorders, including obesity, type 2 diabetes mellitus (T2DM), and metabolic syndrome, continues to rise globally, necessitating interventions that target body composition. Recent advances in surgical, pharmacological, and lifestyle-based therapies have made it increasingly feasible to induce significant metabolic and compositional changes in patients. However, the management of patients post-intervention presents unique clinical challenges, as rapid or profound shifts in fat and lean mass can have wide-reaching physiological, metabolic, and psychosocial consequences. Recent years have seen the advent of novel pharmacological agents—GLP-1 receptor agonists, SGLT2 inhibitors, dual GIP/GLP-1 agonists—that not only facilitate weight loss but also confer cardiometabolic benefits. Consensus guidelines from organizations such as the Endocrine Society, American Diabetes Association (ADA), and International Federation for the Surgery of Obesity (IFSO) underscore the importance of lifelong multidisciplinary follow-up. Key recommendations include regular monitoring of body composition, metabolic parameters, and nutritional status; individualized supplementation protocols; structured physical activity; and proactive screening for complications. Psychosocial support and patient education are also emphasized. Algorithms for risk stratification and management are increasingly informed by large-scale registry data and real-world evidence.
