Iron deficiency anemia (IDA) is the most prevalent nutritional anemia worldwide, frequently encountered in adult clinical practice. Timely and accurate diagnosis is crucial due to its association with significant morbidity and potential for underlying serious pathology, such as gastrointestinal bleeding or malignancy. This review provides an in-depth analysis of the diagnostic approach to IDA in adults, integrating up-to-date clinical guidelines, evidence-based practices, and emerging diagnostic modalities. Mechanistic insights, risk stratification, and practical recommendations for clinicians are discussed, with a focus on optimizing patient outcomes and reducing diagnostic delays.
Iron deficiency anemia is a condition characterized by reduced hemoglobin levels secondary to insufficient iron stores, leading to impaired erythropoiesis. It remains a common reason for medical consultation and laboratory investigation in adults. The clinical approach to diagnosing IDA requires careful consideration of its diverse etiologies, the potential for underlying occult disease, and judicious use of laboratory and endoscopic resources. Given the global burden of IDA and its implications for patient well-being, a structured and evidence-based diagnostic evaluation is essential for effective management.
IDA accounts for a significant proportion of anemia cases in adults, with higher prevalence among women of reproductive age, older adults, and individuals in low- and middle-income regions. According to World Health Organization estimates, over 1.2 billion people worldwide are affected by anemia, with iron deficiency responsible for up to 50% of cases. In developed countries, the prevalence is approximately 2-5% in adult men and postmenopausal women, but may exceed 20% in certain high-risk populations. The disease burden is compounded by the impact of IDA on cognitive and physical function, quality of life, and its association with increased healthcare utilization and mortality, particularly in patients with comorbid conditions.
IDA arises when iron supply is insufficient to meet the demands of erythropoiesis, resulting in the production of hypochromic, microcytic red blood cells. The pathophysiological mechanisms include inadequate dietary intake, impaired absorption (e.g., celiac disease, inflammatory bowel disease), increased physiological requirements (e.g., pregnancy, adolescence), and chronic blood loss (e.g., gastrointestinal or genitourinary sources). Hepcidin, the central regulator of iron homeostasis, plays a pivotal role by modulating intestinal iron absorption and macrophage iron release. In chronic inflammation, elevated hepcidin levels reduce iron availability, further complicating the diagnostic differentiation between absolute and functional iron deficiency.
Common risk factors for IDA in adults include chronic gastrointestinal bleeding (e.g., peptic ulcer disease, colorectal cancer, angiodysplasia), heavy menstrual bleeding, malabsorption syndromes, bariatric surgery, chronic kidney disease, vegetarian or vegan diets, and increased demands due to pregnancy or athletic activity. Recent studies highlight the importance of considering occult gastrointestinal malignancy in at-risk populations, particularly men and postmenopausal women presenting with unexplained IDA.
The clinical manifestations of IDA are heterogeneous, ranging from asymptomatic laboratory findings to severe symptomatic anemia. Common features include fatigue, pallor, exertional dyspnea, palpitations, and reduced exercise tolerance. In more pronounced cases, patients may exhibit glossitis, angular cheilitis, koilonychia (spoon-shaped nails), and restless legs syndrome. Pica, an unusual craving for non-nutritive substances, is a classic but less frequent finding. The chronicity and severity of symptoms often correlate with the degree and rate of hemoglobin decline.
The diagnostic workup for suspected IDA begins with a complete blood count (CBC), typically revealing microcytic, hypochromic anemia. Red cell indices may show low mean corpuscular volume (MCV) and mean corpuscular hemoglobin (MCH). Iron studies are essential for confirmation: serum ferritin (most sensitive and specific marker of iron stores), serum iron, total iron-binding capacity (TIBC), and transferrin saturation (TSAT). Ferritin levels <30 ng/mL are diagnostic in the absence of inflammation. In inflammatory states, a higher threshold (up to 100 ng/mL) is used, and C-reactive protein or erythrocyte sedimentation rate may assist in interpretation. Additional tests—such as reticulocyte hemoglobin content, soluble transferrin receptor, and bone marrow iron staining—are reserved for ambiguous cases. A careful history and physical examination, focusing on potential sources of blood loss and malabsorption, guide further evaluation. Endoscopic assessment of the upper and lower gastrointestinal tract is warranted in adults with unexplained IDA, particularly men and postmenopausal women, to exclude malignancy or other significant pathology.
The cornerstone of IDA management is correction of iron deficiency and identification and treatment of the underlying cause. Oral iron supplementation (e.g., ferrous sulfate, gluconate, or fumarate) is first-line therapy, with typical dosing of 100–200 mg elemental iron daily. Gastrointestinal side effects are common and may necessitate dose adjustment or alternative formulations (e.g., slow-release, liquid, or polysaccharide iron). Parenteral iron (e.g., ferric carboxymaltose, iron sucrose) is indicated in cases of malabsorption, intolerance to oral therapy, chronic kidney disease, or when rapid repletion is required. Response to therapy is assessed by reticulocytosis within 7–10 days and a rise in hemoglobin by 1–2 g/dL over 2–4 weeks. Ongoing monitoring and prevention of recurrence, including dietary counseling and management of chronic blood loss, are essential components of care.
Recent advances in the diagnosis of IDA include the use of hepcidin assays, novel biomarkers (e.g., soluble transferrin receptor-ferritin index), and non-invasive imaging techniques such as MRI-based quantification of liver iron stores. The development of new intravenous iron preparations with improved safety profiles and dosing convenience has expanded therapeutic options, especially in complex cases. Ongoing research focuses on the role of inflammation in iron metabolism, the genetic basis of iron homeostasis, and the potential for targeted therapies to modulate hepcidin activity. These advances promise to refine risk stratification, allow for more individualized treatment, and improve outcomes for patients with refractory or recurrent IDA.
Major clinical guidelines from organizations such as the British Society of Gastroenterology (BSG), American Gastroenterological Association (AGA), and World Health Organization (WHO) emphasize a systematic approach: confirm iron deficiency with laboratory criteria, investigate for underlying causes (especially GI blood loss in at-risk adults), and tailor management to the individual patient\'s clinical context. Routine endoscopic evaluation is recommended for all adult men and postmenopausal women with unexplained IDA, while premenopausal women may undergo selective investigation based on clinical risk factors. Follow-up assessment is critical to ensure hematologic response and resolution of anemia, as well as to detect recurrence and reassess for missed pathology.
Iron deficiency anemia in adults represents a common yet potentially serious clinical entity that necessitates a structured and evidence-based diagnostic approach. Recent advances in laboratory assessment and therapeutic modalities are enhancing clinicians\' ability to accurately diagnose, treat, and monitor this condition. Ongoing vigilance for underlying pathology, particularly gastrointestinal malignancy, remains paramount. Adherence to guideline-based strategies, coupled with individualized patient care, will continue to improve outcomes for adults with IDA.
