Chronic cough, defined as a cough persisting for more than eight weeks, is a common yet perplexing clinical scenario. Despite its prevalence, the etiological understanding and management strategies often pose challenges to clinicians. This article aims to offer a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
Chronic cough can be attributed to a myriad of causes, including but not limited to asthma, gastroesophageal reflux disease (GERD), and postnasal drip syndrome. A thorough patient history and physical examination are paramount in identifying potential triggers. The role of diagnostic tests such as spirometry, chest radiography, and laryngoscopy cannot be understated.
Treatment should be tailored according to the underlying cause. For instance, inhaled corticosteroids are beneficial in cough-variant asthma, while proton pump inhibitors are recommended for GERD-associated cough. However, in cases of unexplained chronic cough, a stepwise therapeutic approach may be employed. This includes empirical treatment trials and specialized investigations if necessary.
Emerging therapies such as neuromodulators and speech therapy have shown promise in the management of chronic cough. Neuromodulators like gabapentin and amitriptyline can be considered in refractory cases. Speech therapy, on the other hand, focuses on behavioral strategies to reduce cough severity and frequency.
Chronic cough is a multifactorial condition requiring a comprehensive approach for effective diagnosis and management. Clinicians should be adept at navigating through the potential etiologies and tailoring treatment accordingly. The incorporation of emerging therapies into the treatment algorithm may offer additional benefits. Continued research and clinical trials are necessary to further elucidate the pathophysiology of chronic cough and improve patient outcomes.
