Integrated kidney care models are increasingly recognized as essential for optimizing outcomes among patients with chronic kidney disease (CKD) and acute kidney injury (AKI). This review examines the scientific rationale, clinical strategies, and emerging evidence supporting seamless care delivery across outpatient and hospital settings. Emphasis is placed on pathophysiology-driven approaches, risk stratification, multidisciplinary management, and the integration of recent guideline recommendations. Practical implications for nephrologists and healthcare teams are discussed, highlighting mechanisms for reducing hospitalizations, improving patient quality of life, and mitigating disease progression.
Chronic kidney disease and acute kidney injury represent a significant and growing public health challenge, with substantial morbidity, mortality, and healthcare costs worldwide. Fragmented care delivery models, where outpatient and inpatient nephrology services operate in silos, often result in suboptimal transitions, delayed interventions, and poorer patient outcomes. Integrated kidney care aims to bridge these gaps by establishing coordinated, patient-centered pathways that encompass prevention, early detection, active management, and effective communication between healthcare providers across different settings. This approach aligns with evolving clinical guidelines and is supported by a growing body of research emphasizing continuity of care and multidisciplinary collaboration.
Globally, CKD affects approximately 10% of the adult population, with prevalence rising due to increased rates of diabetes, hypertension, and aging demographics. Hospitalizations for AKI, which frequently complicate the course of CKD, are associated with increased risk of progression to end-stage renal disease (ESRD) and higher all-cause mortality. In the United States alone, Medicare spending for CKD and ESRD exceeds $120 billion annually, with a significant proportion attributable to potentially preventable hospital admissions and readmissions. The high disease burden underscores the need for integrated care models that address both outpatient and inpatient management to reduce complications and improve resource utilization.
CKD is characterized by progressive nephron loss, maladaptive repair, and persistent inflammation leading to fibrosis and eventual loss of renal function. AKI is defined by a rapid decline in glomerular filtration rate (GFR), often triggered by ischemia, nephrotoxins, or sepsis, and is frequently superimposed on underlying CKD. The interplay between acute and chronic kidney injury is bidirectional, with each episode of AKI conferring incremental risk for CKD progression, while advanced CKD predisposes to AKI due to reduced renal reserve. Mechanistic insights have highlighted the roles of endothelial dysfunction, oxidative stress, and maladaptive immune responses, offering targets for therapeutic intervention.
Major risk factors for CKD include diabetes mellitus, hypertension, cardiovascular disease, obesity, and a family history of kidney disease. Non-modifiable factors such as age, ethnicity (particularly African American and Hispanic populations), and genetic predispositions (e.g., APOL1 variants) also contribute. For AKI, risk factors include advanced age, pre-existing CKD, volume depletion, exposure to nephrotoxic agents, sepsis, and major surgery. Socioeconomic determinants, limited health literacy, and inadequate access to preventive care further amplify risk, highlighting the importance of comprehensive risk assessment as part of integrated care pathways.
CKD often remains asymptomatic until advanced stages, with insidious onset of fatigue, edema, hypertension, and laboratory derangements such as proteinuria and elevated creatinine. AKI presents acutely, with oliguria, azotemia, electrolyte disturbances, and fluid overload. In both conditions, multisystem involvement is common, including cardiovascular, hematologic, and metabolic complications. Early recognition of clinical features, supported by routine screening in high-risk populations, is critical for timely intervention and prevention of adverse outcomes.
Diagnosis of CKD is based on persistent reduction in GFR (<60 mL/min/1.73m² for >3 months) and/or evidence of kidney damage such as albuminuria, structural abnormalities, or histopathological changes. AKI is diagnosed according to KDIGO criteria, which include abrupt increases in serum creatinine or reduction in urine output. Diagnostic workup should include urinalysis, renal imaging, and assessment of comorbidities. Integrated care models emphasize standardized protocols and shared electronic health records to facilitate timely diagnosis and efficient communication between outpatient and inpatient teams.
Management of CKD involves blood pressure and glycemic control, renin-angiotensin-aldosterone system (RAAS) inhibition, dietary modification, and management of complications such as anemia and mineral bone disorder. AKI management focuses on hemodynamic optimization, avoidance of nephrotoxins, timely renal replacement therapy (RRT) when indicated, and supportive care. Integrated models facilitate early nephrology referral, patient education, medication reconciliation, and post-hospitalization follow-up, reducing the risk of rehospitalization and disease progression. Multidisciplinary teams, including dietitians, pharmacists, social workers, and case managers, play a pivotal role in holistic care delivery.
Recent advances in kidney care include the use of sodium-glucose cotransporter-2 (SGLT2) inhibitors, non-steroidal mineralocorticoid receptor antagonists, and novel biomarkers for early AKI detection. Digital health platforms, including telemedicine and remote monitoring, have enhanced continuity of care, particularly during transitions between hospital and home. Risk prediction tools and artificial intelligence (AI)-driven decision support systems are being integrated into clinical practice to aid in early identification of high-risk patients and to personalize therapeutic strategies. Ongoing trials are investigating anti-fibrotic agents, novel immunomodulators, and regenerative therapies that may transform future management paradigms.
Recent guidelines from KDIGO, the National Kidney Foundation, and other authoritative bodies advocate for integrated, multidisciplinary care models emphasizing early identification, risk stratification, and proactive management of kidney disease. Recommendations include routine CKD screening in at-risk populations, prompt nephrology referral for rapid progression or refractory complications, structured care pathways for post-AKI follow-up, and shared decision-making regarding initiation of dialysis or conservative care. Implementation of standardized care bundles and quality improvement initiatives is encouraged to reduce variability and improve outcomes across care settings.
Integrated kidney care across outpatient and hospital settings represents a paradigm shift in the management of CKD and AKI, with robust evidence supporting improved clinical outcomes, reduced hospitalizations, and enhanced patient satisfaction. Multidisciplinary collaboration, adherence to evidence-based guidelines, and adoption of novel therapeutics and digital tools are central to achieving optimal care delivery. Ongoing research and quality improvement efforts will further refine these models, ultimately advancing the standard of care for patients with kidney disease.
