Abstract

Personalized survivorship care in older adults with cancer is an evolving field that addresses the unique clinical needs and complexities faced by this rapidly expanding population. With increasing cancer survival rates, especially among those aged 65 and above, survivorship care has shifted from a one-size-fits-all model to a more nuanced, individualized approach. This review synthesizes the current epidemiology, pathophysiology, risk factors, clinical features, diagnostic challenges, and management strategies in older survivors, highlighting recent advances and evidence-based guideline recommendations to optimize outcomes in this demographic.

Introduction

Cancer survivorship, defined as the health and life of a person with cancer post-treatment, has become a critical focus in oncology, particularly in the geriatric population. Older adults account for the majority of cancer diagnoses and survivorship cases, yet they remain underrepresented in clinical trials and survivorship research. The transition from active treatment to survivorship is fraught with physical, psychological, and social challenges that necessitate individualized care plans. Recognizing the heterogeneity of aging and cancer biology, personalized survivorship care aims to address comorbidities, functional decline, and quality of life while integrating patient preferences and evidence-based interventions.

Epidemiology / Disease Burden

The global population is aging, and with this demographic shift, the burden of cancer survivorship in older adults is increasing. Epidemiological data show that nearly 60% of cancer survivors in developed countries are over 65 years old. Advances in screening, early detection, and treatment have significantly improved survival rates, but aging-related comorbidities and functional impairments complicate post-treatment care. Survivors frequently contend with persistent symptoms such as fatigue, pain, cognitive decline, and increased vulnerability to secondary malignancies, underscoring the necessity for age-specific survivorship strategies.

Pathophysiology

Clinical Features

Diagnosis

Comprehensive geriatric assessment (CGA) is the gold standard, evaluating physical health, cognitive function, psychological wellbeing, social circumstances, and polypharmacy. Biomarkers of aging and frailty, patient-reported outcomes, and functional status assessments are increasingly integrated into survivorship care plans. Timely identification of complications such as cardiotoxicity or osteoporosis is critical for early intervention and prevention of irreversible morbidity.

Treatment & Management

Management strategies for older cancer survivors are inherently personalized, balancing surveillance for recurrence, management of late effects, and optimization of comorbid conditions. Multidisciplinary care teams, including oncology, geriatrics, primary care, rehabilitation, and palliative care, are essential. Interventions may include pharmacologic management of pain and comorbidities, physical therapy for functional decline, cognitive rehabilitation, nutritional support, and psychosocial counseling. Shared decision-making, incorporating patient preferences and goals of care, is fundamental to effective survivorship management.

Recent Advances / Emerging Therapies

Recent advances in personalized survivorship care include the development of geriatric-specific risk assessment tools, digital health interventions for remote monitoring, and integration of precision medicine approaches. Emerging therapies target specific pathways implicated in aging and cancer, such as senolytic drugs and immune modulators. Telemedicine and mobile health applications are increasingly utilized to enhance follow-up care, facilitate early identification of complications, and improve patient engagement, particularly for those with mobility limitations.

Guideline Recommendations

International guidelines, including those from ASCO and NCCN, emphasize the implementation of individualized survivorship care plans for older adults. Recommendations include annual CGA, routine surveillance for recurrence and secondary malignancies, proactive management of comorbidities, and regular assessment of functional and cognitive status. Tailored physical activity programs, vaccination, bone health monitoring, and medication reconciliation are also advocated. Clear communication and care coordination between oncology, primary care, and geriatric providers are critical to guideline adherence and optimal outcomes.

Personalized survivorship care in older adults with cancer is essential for addressing the complexity and heterogeneity of this population. Integrating comprehensive geriatric assessment, individualized risk stratification, and multidisciplinary collaboration fosters improved clinical outcomes, enhances quality of life, and supports healthy aging post-cancer. Ongoing research, innovation in survivorship models, and adherence to guideline-based care are vital to meet the needs of older cancer survivors in clinical practice.
