Enhanced Recovery Surgical Pathways (ERAS) constitute a comprehensive, protocol-driven approach designed to reduce surgical stress, decrease postoperative complications, and accelerate patient recovery. These multidisciplinary pathways integrate best practices across the surgical continuum, encompassing preoperative optimization, intraoperative techniques, and postoperative care. Since their inception in colorectal surgery, ERAS protocols have been expanded to include multiple surgical specialties. However, despite clear benefits demonstrated in the literature, there is considerable heterogeneity in ERAS awareness and adoption globally. Public health initiatives are essential to bridge this knowledge-practice gap, ensuring that both providers and patients benefit from standardized, evidence-based perioperative care.
Globally, millions of surgical procedures are performed annually, with postoperative complications contributing significantly to morbidity, mortality, and healthcare costs. Diagnosis in the context of ERAS primarily involves the systematic identification of modifiable risk factors and barriers to recovery. Preoperative clinic visits serve as critical touchpoints for structured risk assessment, patient education, and shared decision-making. Standardized tools such as the American Society of Anesthesiologists (ASA) Physical Status Classification, frailty indices, and nutritional screening instruments are routinely employed. Postoperative surveillance includes monitoring for deviations from expected recovery trajectories, enabling early intervention and protocol refinement.
ERAS management is inherently multidisciplinary, involving surgeons, anesthesiologists, nurses, physiotherapists, dietitians, and pharmacists. Key components encompass prehabilitation, carbohydrate loading, avoidance of prolonged fasting, intraoperative fluid management, multimodal analgesia, early removal of drains and catheters, and prompt initiation of oral intake and mobilization. Protocol adherence is monitored through audit and feedback mechanisms, with iterative adjustments based on outcome data and patient feedback.
Recent innovations in ERAS include the integration of digital health technologies for real-time monitoring, personalized risk prediction models, and telemedicine-based patient education. Public health strategies for expanding ERAS awareness include professional development workshops, incorporation of ERAS principles into medical curricula, dissemination via professional societies, and leveraging health information systems to support protocol adherence.
