Abstract

Retinal vascular occlusion (RVO), encompassing both central and branch retinal artery and vein occlusions, is increasingly recognized not only as a cause of acute visual loss but also as a harbinger of underlying systemic vascular pathology. This review synthesizes current scientific evidence on the epidemiology, pathophysiology, risk factors, clinical presentation, diagnostic approaches, and management of RVO, while emphasizing its clinical significance as a warning sign of systemic disease. Recent advances in diagnostic imaging and therapeutic interventions are discussed alongside guideline-based recommendations for multidisciplinary care. The article aims to inform clinicians about the necessity of comprehensive systemic assessment and coordinated management in patients presenting with RVO, highlighting its prognostic and preventive implications.

Introduction

Retinal vascular occlusion represents a spectrum of vascular disorders affecting the retinal arteries or veins, leading to sudden, painless vision loss. While the primary ocular manifestation is well-recognized, an increasing body of evidence highlights RVO as a sentinel event for systemic vascular diseases, notably cardiovascular and cerebrovascular disorders. The retina, as an extension of the central nervous system, offers a unique opportunity for in vivo visualization of the microvasculature, providing critical insights into systemic vascular health. This review explores the clinical and scientific underpinnings of RVO, its epidemiological significance, and the necessity for an integrated approach to diagnosis and management.

Epidemiology / Disease Burden

Retinal vascular occlusions are among the most common vascular disorders of the eye, with branch retinal vein occlusion (BRVO) and central retinal vein occlusion (CRVO) being the predominant types. The global prevalence of RVO is estimated at 5.20 per 1,000 persons for BRVO and 0.80 per 1,000 for CRVO, increasing with age and comorbidities. Retinal artery occlusions (RAO), including central and branch variants, are less common but carry a higher risk of subsequent systemic vascular events. The burden of RVO is substantial, leading not only to significant visual morbidity but also to increased healthcare resource utilization owing to the need for systemic evaluation and management. Epidemiological data underscore the crucial association between RVO and the risk of myocardial infarction, stroke, and peripheral arterial disease, warranting heightened vigilance among healthcare professionals.

Pathophysiology

The pathogenesis of retinal vascular occlusion is multifactorial, involving a complex interplay of systemic and local factors. In vein occlusions, Virchow\"s triad of stasis, endothelial injury, and hypercoagulability is central, with arteriovenous crossing points being common sites due to mechanical compression. Arterial occlusions typically result from embolic phenomena, often originating from carotid atherosclerosis, cardiac valvular disease, or hypercoagulable states. Both forms of occlusion lead to retinal ischemia, breakdown of the blood-retinal barrier, and subsequent neovascularization, increasing the risk for secondary complications such as neovascular glaucoma. Recent molecular insights implicate inflammatory pathways, oxidative stress, and dysregulation of angiogenic factors, notably vascular endothelial growth factor (VEGF), in the progression and sequelae of RVO.

Risk Factors

Major risk factors for RVO mirror those for systemic vascular disease and include advanced age, hypertension, diabetes mellitus, dyslipidemia, and smoking. Additional systemic contributors include hypercoagulable states, such as antiphospholipid syndrome, hyperhomocysteinemia, and inherited thrombophilias. Ocular factors, including glaucoma and increased intraocular pressure, are particularly relevant for vein occlusions. The identification of risk factors is critical, not only for targeted management but also for the detection of underlying systemic conditions that may otherwise remain occult until catastrophic events occur.

Clinical Features

The clinical presentation of RVO is typically characterized by sudden, painless, unilateral vision loss or visual field defect. Fundoscopic findings in CRVO include diffuse retinal hemorrhages, dilated tortuous veins, and optic disc swelling, while BRVO presents with sectoral involvement. Arterial occlusions manifest as retinal whitening, a cherry-red spot, and arteriolar attenuation. Ocular complications include macular edema, retinal neovascularization, and vitreous hemorrhage. Importantly, the detection of RVO should prompt a thorough systemic evaluation to identify coexisting vascular risk factors or thromboembolic sources.

Diagnosis

Diagnosis of RVO is primarily clinical, supported by fundus examination and multimodal imaging. Optical coherence tomography (OCT) is invaluable for detecting macular edema, while fluorescein angiography delineates areas of capillary non-perfusion and neovascularization. For arterial occlusions, carotid Doppler ultrasonography and echocardiography are essential to evaluate embolic sources. Laboratory investigations should include assessment for blood dyscrasias, hypercoagulable states, and systemic vascular risk profiling. Timely and accurate diagnosis is imperative, as it influences both ocular prognosis and systemic risk stratification.

Treatment & Management

The management of RVO is multifaceted, targeting both ocular complications and systemic risk factors. Intravitreal anti-VEGF agents are the mainstay for vision-threatening macular edema, with corticosteroids reserved for select cases. Laser photocoagulation is indicated for neovascularization. Systemic management entails aggressive control of hypertension, diabetes, and dyslipidemia, as well as antiplatelet or anticoagulant therapy in cases with identifiable thromboembolic risk. Collaboration with internists, cardiologists, and hematologists is crucial for comprehensive care and secondary prevention.

Recent Advances / Emerging Therapies

Recent years have witnessed significant progress in the management of RVO. Sustained-release corticosteroid implants and novel anti-VEGF agents offer improved efficacy and convenience. Advances in imaging, such as OCT angiography, facilitate earlier detection of ischemia and neovascularization. Ongoing research explores the role of neuroprotective agents, combination therapies, and systemic anti-inflammatory drugs. There is growing interest in personalized medicine approaches, leveraging genetic and biomarker profiling to refine risk assessment and therapeutic choices.

Guideline Recommendations

International and national guidelines increasingly emphasize the dual focus of RVO management: prompt treatment of ocular complications and systematic evaluation of systemic vascular risk. The American Academy of Ophthalmology and European Society of Retina Specialists recommend baseline systemic assessment for all patients with RVO, including blood pressure, glycemic, and lipid profiling. For arterial occlusions, urgent neurovascular imaging is advocated to mitigate the risk of stroke. Long-term follow-up with multidisciplinary teams is advised to optimize visual and systemic outcomes.

Retinal vascular occlusion serves as a critical warning sign of systemic vascular disease, necessitating a paradigm shift in clinical practice. Early recognition, comprehensive systemic evaluation, and coordinated management are essential to improve both ocular and systemic prognoses. With emerging therapies and evolving guidelines, a multidisciplinary approach remains the cornerstone of effective care, underscoring the pivotal role of ophthalmologists and healthcare professionals in the prevention of future vascular events.
