Healthy aging is a multidimensional process influenced by an array of factors beginning as early as childhood. Contemporary research demonstrates that determinants spanning genetics, environment, socioeconomic status, nutrition, early-life exposures, and psychosocial development contribute to the risk or resilience for chronic diseases, functional decline, and mortality in later life. This review critically examines epidemiological trends, mechanistic insights, risk stratification, and clinical implications of early-life determinants on the trajectory of healthy aging, integrating recent evidence and expert guideline recommendations for healthcare professionals.
The concept of healthy aging encompasses not merely the absence of disease, but also the maintenance of physical, psychological, and social well-being throughout the lifespan. Increasing evidence underscores that the origins of healthy or pathological aging are rooted in the earliest stages of life, with exposures and interventions in childhood exerting profound influences on adult health outcomes. For clinicians and public health professionals, understanding these determinants is essential for proactive, life-course approaches to disease prevention and health promotion.
Globally, the demographic shift towards an aging population presents considerable challenges. According to the World Health Organization, by 2050, one in six people will be over age 65. The prevalence of non-communicable diseases (NCDs) such as cardiovascular disease, diabetes, and neurodegenerative disorders escalates with age, contributing to substantial morbidity, mortality, and healthcare costs. Epidemiologic studies, including large longitudinal cohorts, have demonstrated that childhood adversity, malnutrition, and insufficient early psychosocial support are associated with a higher burden of NCDs, increased frailty, and impaired quality of life in old age. Conversely, protective factors in childhood correlate with increased longevity and reduced disability-adjusted life years (DALYs).
The biological embedding of early-life experiences is increasingly recognized as central to aging trajectories. Mechanistically, suboptimal nutrition, chronic stress, exposure to toxins, and adverse childhood events (ACEs) can induce epigenetic modifications, alter hypothalamic-pituitary-adrenal (HPA) axis regulation, and increase systemic inflammation. These changes contribute to accelerated cellular senescence, impaired immune function, and metabolic dysregulation. The Developmental Origins of Health and Disease (DOHaD) hypothesis further elucidates how fetal and early postnatal environments program organ systems, influencing susceptibility to hypertension, insulin resistance, and neurocognitive decline decades later.
Multiple risk factors in childhood have been implicated in poor aging outcomes. These include low birth weight, undernutrition or overnutrition, exposure to environmental toxins (e.g., lead, air pollution), low socioeconomic status, limited access to healthcare, sedentary lifestyle, and psychosocial stressors such as family instability or abuse. Genetic predisposition interacts with these environmental exposures, modulating individual vulnerability. Importantly, cumulative risk—where multiple adverse factors coexist—has a compounding effect on age-related morbidity.
While the consequences of early-life determinants often manifest decades later, subtle clinical markers may emerge in adolescence or early adulthood. These include impaired growth trajectories, early signs of metabolic syndrome, hypertension, reduced cognitive or psychosocial resilience, and subclinical organ dysfunction. Recognizing these early clinical features allows for timely intervention, potentially altering the life-course risk for chronic diseases and disability.
Diagnosis of at-risk individuals requires a comprehensive, multidimensional approach. Detailed social and medical histories capturing prenatal exposures, nutritional status, family environment, and psychosocial experiences are critical. Biomarkers such as inflammation (e.g., C-reactive protein), metabolic markers (e.g., HbA1c, lipid profile), and, increasingly, epigenetic or transcriptomic signatures may offer additional risk stratification. Screening for developmental delays, growth abnormalities, and early mental health disorders provides actionable insights for clinicians.
Effective management begins with early identification and mitigation of modifiable risk factors. Interventions encompass optimizing maternal and child nutrition, promoting physical activity, ensuring immunization, and providing psychosocial support. For children identified at higher risk, multidisciplinary management—including pediatric, psychiatric, nutritional, and social work input—may be required. In adulthood, ongoing surveillance and management of risk factors such as obesity, hypertension, and dysglycemia are essential to prevent downstream morbidity.
Recent research highlights novel interventions such as targeted pre- and postnatal supplementation (e.g., omega-3 fatty acids, micronutrients), school-based health promotion programs, and early-life stress reduction strategies. Advances in epigenetic therapeutics and personalized medicine may enable risk-tailored interventions. Digital health and artificial intelligence are increasingly used to monitor growth, cognitive development, and psychosocial well-being, allowing for earlier and more precise intervention.
Major organizations, including the WHO and American Academy of Pediatrics, advocate for integrated, life-course approaches to health promotion. Guidelines emphasize the importance of maternal health optimization, breastfeeding, balanced nutrition, regular physical activity, mental health screening, and reduction of environmental exposures from infancy onward. Implementing these recommendations requires coordinated efforts among healthcare providers, families, schools, and policy-makers.
A growing body of evidence confirms that the determinants of healthy aging originate in childhood, with lifelong consequences for individual and population health. Clinicians must adopt a proactive, multidisciplinary approach, emphasizing early intervention and risk mitigation. Future research will further elucidate the mechanisms linking early-life exposures with aging, and emerging therapies hold promise for reshaping aging trajectories. Commitment to early-life health promotion is essential for fostering resilient, healthy aging across generations.
