Bariatric surgery, including procedures such as Roux-en-Y gastric bypass and sleeve gastrectomy, has demonstrated significant efficacy in achieving substantial and sustained weight loss among individuals with morbid obesity. The rising prevalence of obesity-related complications, including type 2 diabetes, cardiovascular disease, and non-alcoholic fatty liver disease, has intensified the demand for bariatric interventions. However, the complexity of patient profiles mandates thorough preoperative diagnostic evaluation to minimize surgical risks and maximize therapeutic benefits. This article provides a detailed, evidence-based overview tailored for clinicians involved in the management of candidates for bariatric surgery.
Obesity is a global public health concern, affecting over 650 million adults worldwide, according to the World Health Organization (WHO). In the United States, more than 40% of adults meet the criteria for obesity, with a substantial proportion eligible for bariatric intervention based on BMI thresholds and the presence of comorbidities. The disease burden is considerable, as obesity is associated with increased morbidity, healthcare utilization, and premature mortality. The growing epidemic necessitates effective therapeutic strategies and careful patient selection for surgical interventions.
The pathophysiology of obesity is multifactorial, involving genetic, neurohormonal, and environmental determinants. Major societies, including the American Society for Metabolic and Bariatric Surgery (ASMBS) and the National Institutes of Health (NIH), endorse a structured, multidisciplinary evaluation. Recommendations emphasize comprehensive metabolic screening, cardiopulmonary assessment, upper gastrointestinal endoscopy, psychological evaluation, and education regarding perioperative risks and lifestyle changes. Candidates must meet established BMI and comorbidity criteria, and contraindications such as active substance abuse, untreated psychiatric illness, or severe organ dysfunction must be addressed prior to proceeding with surgery.
A meticulous preoperative diagnostic evaluation is imperative for the safe and effective delivery of bariatric surgery. The integration of clinical, laboratory, and psychosocial assessments ensures optimal patient selection, risk mitigation, and multidisciplinary coordination. Adherence to evidence-based guidelines, incorporation of novel diagnostic modalities, and individualized care pathways represent best practices in the pre-bariatric surgical workup.
