Introduction

Parkinson's disease (PD) is a neurodegenerative disorder affecting the motor system, characterized by tremors, rigidity, bradykinesia, and postural instability. It is the second most common neurodegenerative disease, affecting approximately 1% of the population over 60 years old.

Etiology and Pathophysiology

PD is caused by the loss of dopaminergic neurons in the substantia nigra pars compacta (SNpc). The exact cause of this neuronal loss is unknown, but it is believed to involve a combination of genetic and environmental factors. Accumulation of alpha-synuclein, forming Lewy bodies, is a hallmark of PD, contributing to neuronal death.

Clinical Presentation

PD typically presents with asymmetric resting tremor, rigidity, and bradykinesia. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, and mood disorders can also occur. The disease progresses gradually, and diagnosis can be challenging in the early stages.

Diagnosis

Diagnosis of PD is primarily clinical, based on the presence of cardinal motor symptoms and response to dopaminergic therapy. Neuroimaging can aid in differential diagnosis but is not routinely used. Currently, there are no definitive biomarkers for PD.

Treatment

Treatment of PD is symptomatic, aiming to improve quality of life. Pharmacological treatment primarily involves dopamine replacement with levodopa. Other drugs, such as dopamine agonists, MAO-B inhibitors, and COMT inhibitors, can also be used. Deep brain stimulation is an option for patients with advanced disease or those who do not respond to medication.

Conclusion

Understanding Parkinson's disease is crucial for its early recognition and appropriate management. Although there is no cure, symptomatic treatment can significantly improve the quality of life of patients. Ongoing research aims to uncover the pathophysiology of PD to develop more effective treatments and preventive strategies.
