Enteric fever, commonly caused by Salmonella Typhi and Paratyphi, continues to be a significant global health issue, particularly in developing countries. The management of this condition has been challenged by rising antimicrobial resistance, necessitating advancements in diagnostic methods and treatment strategies.
Traditional culture methods, while remaining the gold standard, are laborious and time-consuming. Rapid diagnostic tests (RDTs) have emerged as a promising tool, offering results in less than 15 minutes. However, their sensitivity and specificity vary. Polymerase chain reaction (PCR) based assays have shown high sensitivity and specificity, but their use is limited by cost and technical requirements. Metagenomic next-generation sequencing (mNGS) is a promising tool that can detect a wide range of pathogens, but further research is needed to establish its role in diagnosing enteric fever.
Antimicrobial resistance has necessitated a shift from first-line drugs like chloramphenicol, ampicillin, and trimethoprim-sulfamethoxazole to fluoroquinolones. However, resistance to fluoroquinolones is also emerging. Therefore, third-generation cephalosporins and azithromycin have become the treatment of choice. Vaccination also plays a critical role in prevention, with the typhoid conjugate vaccine (TCV) showing promising results in controlling the spread of the disease.
Clinical guidelines are instrumental in ensuring effective management of enteric fever. The World Health Organization (WHO) provides guidelines on diagnosis and treatment, including the use of newer diagnostic tools and antimicrobial therapy. Adherence to these guidelines is crucial for optimal patient outcomes.
In the face of rising antimicrobial resistance, advancements in the diagnosis and treatment of enteric fever are crucial. Rapid diagnostic tests, PCR-based assays, and mNGS offer potential for improved diagnosis. Treatment strategies are evolving, with a shift towards third-generation cephalosporins and azithromycin. Vaccination and adherence to clinical guidelines are paramount in managing this global health issue.
