Pulmonary hypertension (PH) is a complex and often misunderstood condition. This article aims to provide a comprehensive overview to enhance understanding and management of PH in clinical practice.
PH is defined as an increase in mean pulmonary arterial pressure ≥ 25 mmHg at rest. The World Health Organization (WHO) classifies PH into five groups based on etiology, clinical presentation, hemodynamic characteristics, and treatment strategies.
The pathophysiology of PH is multifactorial and involves vasoconstriction, vascular remodeling, inflammation, and in situ thrombosis. The result is elevated pulmonary vascular resistance, leading to right ventricular failure and ultimately death if untreated.
Patients with PH often present with non-specific symptoms such as dyspnea, fatigue, and chest pain. Diagnosis requires a high index of suspicion and is confirmed by echocardiography and right heart catheterization. Additional tests help to determine the underlying cause of PH.
Treatment of PH is primarily aimed at the underlying cause. Vasodilators, endothelin receptor antagonists, phosphodiesterase-5 inhibitors, and prostacyclin analogs are used to manage symptoms and slow disease progression. In severe cases, lung transplantation may be considered.
The prognosis of PH is generally poor, with median survival rates of 2.8 years without treatment. However, early diagnosis and appropriate treatment can significantly improve survival and quality of life.
In conclusion, PH is a complex disease requiring a multidisciplinary approach for optimal management. Understanding its fundamentals, from pathophysiology to treatment, is crucial for healthcare professionals to provide the best possible care to patients with this challenging condition.
