Enteric fever, commonly known as typhoid fever, is a systemic infection primarily caused by Salmonella Typhi and Paratyphi. Despite significant improvements in public health measures, it continues to pose a significant health burden, particularly in developing nations. This article aims to provide a comprehensive guide on the recent advancements in the diagnosis and treatment strategies for enteric fever.
Traditionally, blood culture has been the gold standard for diagnosing enteric fever. However, it has limitations such as low sensitivity and a long turnaround time. Recent advancements include the introduction of molecular techniques like Polymerase Chain Reaction (PCR) and Nucleic Acid Amplification Tests (NAATs). These offer higher sensitivity and specificity, and quicker results. Additionally, rapid diagnostic tests (RDTs) using immunochromatographic methods have shown promising results in preliminary studies.
Treatment of enteric fever has been complicated by the emergence of multi-drug resistant (MDR) strains. The current treatment strategy involves the use of fluoroquinolones, third-generation cephalosporins, and azithromycin. Recent studies indicate the promising role of newer agents like gatifloxacin and cefixime. Adjunctive therapies such as probiotics and zinc supplementation have also shown potential in reducing the disease burden.
Prevention is always better than cure, and in the case of enteric fever, vaccination plays a crucial role. The two vaccines currently available are the Vi polysaccharide vaccine and the Ty21a vaccine. Recent developments include the introduction of a new conjugate vaccine (TCV), which has shown higher efficacy and longer duration of protection.
While strides have been made in diagnosing and treating enteric fever, the disease continues to be a significant public health issue, particularly in resource-limited settings. The advent of rapid diagnostic tests, newer antimicrobial agents, and more effective vaccines offers hope for better management and control of this disease. However, further research is needed to improve these strategies and develop new ones.
