Enteric fever, predominantly caused by Salmonella Typhi and Paratyphi, remains a significant global health concern. The complexity of diagnosis and evolving resistance patterns necessitate updated approaches for healthcare professionals.
Traditional culture methods, though gold standard, have limitations in sensitivity and speed. Recent advancements include serologic tests like Typhidot and Tubex TF, offering quicker results. However, their variable sensitivity and specificity necessitate cautious interpretation. Molecular diagnostic methods, such as PCR, show promise with high sensitivity and specificity, but require further validation.
Antimicrobial resistance complicates treatment. Fluoroquinolones, previously first-line, are now less effective due to widespread resistance. Current WHO guidelines recommend empirical treatment with ceftriaxone or azithromycin. Vaccination, though not 100% effective, remains an important preventive measure, especially in endemic areas.
Healthcare professionals play a critical role in diagnosing enteric fever early, initiating appropriate treatment, and preventing disease spread. Continued education about updated diagnostic methods and treatment guidelines is essential.
Enteric fever continues to pose significant challenges in diagnosis and treatment. Embracing new diagnostic tools, adhering to updated treatment guidelines, and promoting vaccination are key strategies in managing this global health issue. Healthcare professionals' adaptability to these changes is critical in reducing the burden of enteric fever.
