Chronic cough, defined as a cough persisting beyond eight weeks, presents a diagnostic challenge to clinicians due to its multi-factorial etiology. It can significantly impair quality of life, causing physical, social, and psychological distress. This article provides a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
The initial evaluation of a patient with chronic cough should include a detailed history and physical examination to identify common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS). Diagnostic tests should be guided by the patient's history and physical examination. In cases where common causes are ruled out, more extensive testing may be required to identify less common causes such as non-asthmatic eosinophilic bronchitis, bronchiectasis, or neoplasms.
Treatment should primarily target the underlying cause of the cough. For example, proton pump inhibitors may be prescribed for GERD, inhaled corticosteroids for asthma, and antihistamines for UACS. If the cough persists despite treatment, it may be classified as a refractory chronic cough. In these cases, a multidisciplinary approach involving pulmonologists, gastroenterologists, and otolaryngologists may be beneficial. Non-pharmacological interventions such as speech and language therapy can also be considered.
Recent research has highlighted the role of neuronal hypersensitivity in the pathogenesis of chronic cough. New therapies targeting neuronal pathways such as P2X3 receptor antagonists and transient receptor potential (TRP) channel modulators are currently under investigation and may offer new hope for patients with refractory chronic cough.
In conclusion, chronic cough is a complex medical condition that requires a thorough diagnostic evaluation and a targeted treatment approach. The management of refractory cases can be challenging, but emerging therapies offer promising avenues for future treatment. Continued research and collaboration across specialties are essential for advancing our understanding and management of this debilitating condition.
