This review synthesizes current evidence on the epidemiology, mechanisms, clinical features, and management strategies for body composition shifts in women, with a focus on practical, guideline-driven recommendations for clinicians.

Understanding the trajectory of body composition changes in women across reproductive aging is critical for optimizing preventive and therapeutic strategies in clinical practice. While early adulthood is characterized by relative stability in fat and lean mass, the peri-menopausal and post-menopausal periods are marked by accelerated fat accumulation, redistribution, and loss of muscle mass (sarcopenia). These changes are intertwined with metabolic and cardiovascular risk profiles, necessitating a nuanced, evidence-based approach to women's health that extends beyond traditional reproductive concerns.

Epidemiology / Disease Burden

Large-scale epidemiological studies demonstrate that women experience a progressive increase in total body fat, particularly visceral adipose tissue, beginning in the perimenopausal years and extending into late life. Treatment & Management

Management strategies are multifaceted and should be individualized. Lifestyle interventions remain the cornerstone, with strong evidence supporting the benefits of resistance and aerobic exercise in preserving lean mass and reducing visceral adiposity. Dietary interventions focusing on adequate protein intake, caloric balance, and micronutrient sufficiency (notably calcium and vitamin D) are essential. Pharmacologic therapy may be considered for women with obesity-related comorbidities (e.g., GLP-1 receptor agonists) or osteoporosis (e.g., bisphosphonates, selective estrogen receptor modulators). Hormone replacement therapy may be appropriate for selected women with significant menopausal symptoms and low cardiovascular risk, but requires careful risk-benefit assessment.

Recent Advances / Emerging Therapies

Recent research has highlighted the potential role of novel agents such as myostatin inhibitors, selective androgen receptor modulators (SARMs), and mitochondrial-targeted antioxidants in mitigating muscle loss and fat gain in aging women. Advances in precision medicine, including genetic risk stratification and individualized exercise prescription, offer promise for more tailored interventions. Ongoing trials are investigating the safety and efficacy of combined lifestyle and pharmacological approaches to optimize body composition and metabolic health in postmenopausal women.

Guideline Recommendations

Major guidelines, including those from the North American Menopause Society (NAMS) and Endocrine Society, emphasize routine screening for body composition changes in midlife women, early intervention for overweight/obesity, and the integration of exercise and nutrition counseling into routine care. The International Osteoporosis Foundation recommends periodic assessment of bone density and fall risk in women over 50. Hormone therapy should be considered on an individualized basis, weighing symptomatic benefit against cardiovascular and oncologic risk profiles.
