Emphasis is placed on accurate diagnosis, risk factor identification, individualized treatment planning, recent advances, and practical clinical implementation to optimize patient-centered care.
Tooth wear is a multifactorial condition characterized by the progressive loss of dental hard tissues due to mechanisms other than caries or trauma. Severe wear can compromise aesthetics, function, and patient quality of life, necessitating multidisciplinary and often complex rehabilitation. The increase in patient longevity and retention of natural teeth into older age underscores the need for robust, evidence-based management protocols. This article provides clinicians with an up-to-date, practical framework for case-based rehabilitation of severe tooth wear, drawing from current literature, consensus guidelines, and clinical experience.
The prevalence of severe tooth wear has been reported to range from 3% to 17% in adult populations, with higher rates observed in elderly cohorts due to cumulative exposure to risk factors. The Global Burden of Disease Study highlights tooth wear as a significant contributor to oral disability, with increasing incidence attributed to dietary, behavioral, and iatrogenic factors. Restorative options range from direct composite build-ups and adhesive onlays for localized wear to full-arch prosthetic rehabilitation in advanced cases. Vertical dimension restoration requires careful planning, incremental increases, and reversible trials to ensure patient adaptation. In cases with significant tooth loss, implant-supported prostheses may be considered. Long-term maintenance protocols, including regular recall, reinforcement of preventive strategies, and repair of restorations, are essential to ensure lasting outcomes.
Recent advances in adhesive dentistry, CAD/CAM technologies, and biomimetic materials have revolutionized the rehabilitation of severe tooth wear. Minimally invasive approaches, such as the use of high-strength composite resins and lithium disilicate ceramics, allow for preservation of remaining tooth structure while restoring function and aesthetics. Digital smile design and virtual articulation facilitate precise planning and patient communication. Novel splint materials and telemonitoring of bruxism represent emerging adjuncts to risk management. The European Federation of Conservative Dentistry and related bodies advocate for individualized care plans, supported by detailed documentation, patient education, and interdisciplinary collaboration. Regular follow-up and outcome monitoring are critical elements of successful long-term management. Informed consent and shared decision-making are integral throughout the rehabilitative process.
Effective rehabilitation of severe tooth wear demands a comprehensive, evidence-based, and patient-centered approach. Advances in diagnostic techniques, materials, and digital workflows have expanded the therapeutic armamentarium, enabling clinicians to address both functional and aesthetic concerns with greater predictability. Ongoing research and guideline evolution continue to refine best practices, underscoring the importance of vigilance, prevention, and individualized care in achieving optimal outcomes for patients with severe tooth wear.
