While clinical interventions at the individual level are critical, population-based preventive programs are integral to reducing disease incidence, morbidity, and social disparities. Community-based oral health initiatives encompass coordinated efforts—such as water fluoridation, school-based sealant application, oral health education, and access to preventive dental care—that target modifiable risk factors and promote sustained behavioral change. These programs are especially valuable in underserved communities where barriers to routine dental care and health literacy persist. This review explores the scientific foundations, mechanisms, and practical applications of community-based oral disease prevention programs, with an emphasis on evidence-based and guideline-driven practice.
Oral diseases constitute a significant public health issue, with the Global Burden of Disease Study 2019 estimating that 3.5 billion people are affected by oral conditions worldwide. Dental caries is the most common chronic childhood disease, and severe periodontal disease is a leading cause of tooth loss in adults. Disparities in oral health outcomes are pronounced among populations with limited access to dental care, lower socioeconomic status, and minority backgrounds. Community-based preventive programs have demonstrated measurable reductions in caries prevalence and improvements in oral hygiene indicators across diverse settings, underscoring their critical role in addressing this burden.

Management strategies within community-based programs emphasize primary prevention through fluoride varnish application, fissure sealants, supervised toothbrushing, and oral health education. Implementation should be guided by local epidemiological data, resource availability, and continuous quality improvement processes.
