Sustainable weight management necessitates a comprehensive public health approach that extends beyond individual interventions and addresses environmental, social, and systemic determinants. This review synthesizes current evidence on public health strategies for creating supportive environments for weight management, with a focus on epidemiology, pathophysiology, risk factor modification, diagnostic frameworks, clinical management, emergent therapies, and guideline-based recommendations. Emphasis is placed on integrating clinical and public health measures to optimize outcomes and promote long-term, population-level success.
The prevalence of overweight and obesity has reached epidemic proportions globally, constituting one of the most significant public health threats of the 21st century. The etiology of weight gain is multifactorial, involving genetic, behavioral, metabolic, and environmental contributors. Traditional management has focused on individualized behavioral interventions; however, mounting evidence underscores the necessity of shifting toward systems-based, sustainable approaches. This review examines the scientific basis and practical implications of public health strategies aimed at cultivating environments conducive to healthy weight maintenance, with a focus on the integration of clinical and population-level interventions for maximal impact.
According to the World Health Organization (WHO), over 1.9 billion adults were overweight in 2016, with 650 million classified as obese. The rising trajectory is mirrored in pediatric and adolescent populations, forecasting an increased burden of obesity-related comorbidities such as type 2 diabetes, cardiovascular disease, and certain cancers. The attributable healthcare expenditures and loss of productivity pose immense societal costs, especially in low- and middle-income countries experiencing rapid urbanization and dietary transitions. Disparities in obesity prevalence persist across sociodemographic strata, highlighting the role of environmental and structural determinants in shaping population health outcomes.
The regulation of body weight involves a complex interplay between neuroendocrine, metabolic, and behavioral mechanisms. Disruption of homeostatic pathways—such as leptin and insulin signaling—contributes to altered appetite control, reduced energy expenditure, and enhanced adipogenesis. Environmental factors, including the availability of energy-dense foods and reduced opportunities for physical activity, potentiate these intrinsic susceptibilities. The clustering of obesogenic risk factors within families and communities emphasizes the importance of collective and upstream interventions.
Obesity presents with a spectrum of clinical features, from asymptomatic excess adiposity to overt comorbidities including insulin resistance, hypertension, dyslipidemia, obstructive sleep apnea, and non-alcoholic fatty liver disease. Physical findings may include centripetal fat distribution, acanthosis nigricans, and joint pain. Psychological sequelae, such as depression and reduced quality of life, are prevalent and impact treatment adherence. Recognition of these features is essential for risk stratification and guiding individualized management within the context of broader public health strategies.
The diagnosis of overweight and obesity is primarily based on anthropometric measurements, with body mass index (BMI) serving as the standard tool. Waist circumference and waist-to-hip ratio provide additional risk stratification for cardiometabolic complications. These strategies aim to shift population behaviors by modifying the environments in which choices are made, thus complementing clinical management with broad-based prevention.
Emerging therapies for obesity include novel pharmacological agents targeting appetite regulation, gut hormones, and energy metabolism. Digital health technologies—such as mobile applications, wearable devices, and telemedicine platforms—have demonstrated promise in supporting behavior change and expanding access to care. Community-level interventions leveraging social networks and behavioral economics are being explored for their potential to drive collective action and sustainable change. There is a growing emphasis on implementation science to bridge evidence-practice gaps and optimize the scalability of effective interventions.
Recent guidelines from organizations such as the WHO, Centers for Disease Control and Prevention (CDC), and national obesity societies emphasize a comprehensive, multisectoral approach to weight management. Key recommendations include the integration of obesity prevention into all relevant policies, creation of healthy food environments, promotion of physical activity through urban design, and reduction of socioeconomic barriers to health. Clinicians are encouraged to advocate for policy change, participate in community partnerships, and adopt a life-course approach to obesity prevention and management.
Sustainable weight management hinges on the creation of health-promoting environments that support individual and collective efforts to maintain healthy body weight. Public health strategies must address the complex interplay of biological, behavioral, and environmental determinants, leveraging evidence-based interventions at policy, community, and clinical levels. Ongoing research, innovation, and cross-sector collaboration are essential to reversing current trends and improving population health outcomes. Healthcare professionals play a pivotal role as advocates, educators, and leaders in shaping environments that foster lifelong healthy weight management.
