This review critically evaluates the role of comprehensive geriatric assessment (CGA) prior to initiation of hematologic cancer therapy, focusing on its epidemiological significance, underlying mechanisms, risk stratification, and clinical implications. The discussion synthesizes recent evidence, guideline recommendations, and emerging tools that inform personalized treatment strategies, aiming to optimize patient selection, mitigate risks, and improve clinical outcomes.
Hematologic malignancies, including leukemia, lymphoma, and myeloma, disproportionately affect the geriatric population. The increasing prevalence of cancer in aging societies underscores the necessity for nuanced pre-therapy evaluation in older adults. Comprehensive geriatric assessment extends beyond standard oncology evaluation, encompassing multidimensional domains such as functional status, comorbidities, cognition, psychological state, social support, nutrition, and polypharmacy. This holistic approach facilitates risk stratification, individualized therapy, and proactive management of vulnerabilities, aligning with contemporary oncology principles of precision medicine.
The median age at diagnosis for most hematologic cancers exceeds 65 years. Epidemiological data from population-based registries demonstrate that over 60% of new cases of non-Hodgkin lymphoma, acute myeloid leukemia (AML), and multiple myeloma occur in patients over 65. The cancer burden in older adults is compounded by age-associated comorbidities, physiological decline, and socioeconomic disparities, often resulting in under-treatment and poorer outcomes compared to younger cohorts. The rising incidence of hematologic neoplasms among the elderly amplifies the clinical and economic impact, necessitating tailored assessment frameworks for this group.
Aging is accompanied by progressive changes in hematopoietic and immune systems, organ function, and cellular resilience. Geriatric assessment elucidates the interplay between tumor- and host-related factors, enabling identification of intrinsic vulnerabilities that influence therapy tolerability and recovery.
Key risk factors affecting therapeutic outcomes in older adults with hematologic malignancies include frailty, comorbid chronic illnesses (such as cardiovascular disease, diabetes, and renal impairment), cognitive decline, poor nutritional status, polypharmacy, and limited social support. Frailty, a multidimensional syndrome defined by diminished physiological reserve, has emerged as a robust independent predictor of treatment-related toxicity, hospitalization, and mortality. Geriatric assessment tools systematically quantify these risk factors to guide clinical decision-making.
Clinical presentation in geriatric patients may be atypical or masked by concurrent comorbidities. Symptoms such as fatigue, weight loss, and infections are often attributed to aging rather than underlying malignancy. Furthermore, age-related changes in pharmacology and organ function increase the risk of adverse events. Geriatric assessment enables early identification of subtle clinical features, functional decline, and pre-existing impairments, thereby informing prognosis and intervention strategies.
Diagnostic evaluation in older adults should integrate standard oncologic workups (histopathology, immunophenotyping, cytogenetics, and imaging) with CGA-derived insights. Assessment of functional and cognitive status, mobility, mental health, and nutritional parameters is essential for establishing baseline vulnerability and anticipating therapy-related complications. Validated instruments such as the Geriatric 8 (G8), Vulnerable Elders Survey-13 (VES-13), and the Comprehensive Geriatric Assessment itself are routinely employed in clinical trials and practice to stratify risk.
Therapeutic regimens for hematologic malignancies in older adults must be individualized based on CGA findings. Fit patients may benefit from standard-intensity protocols, while those with frailty or significant comorbidities may require dose modifications or alternative approaches, including targeted therapies or supportive care. Multidisciplinary collaboration among hematologists, geriatricians, pharmacists, and allied health professionals is critical for optimizing therapy, managing polypharmacy, and addressing psychosocial needs. Early identification of risks enables pre-emptive interventions, such as physical rehabilitation, nutritional support, and cognitive stimulation, to enhance resilience.
Innovative therapeutic strategies, such as novel immunotherapies, targeted agents (e.g., BTK inhibitors, BCL-2 inhibitors), and reduced-intensity conditioning for hematopoietic stem cell transplantation, have expanded the treatment landscape for older adults. Recent studies validate the feasibility and efficacy of these modalities in selected geriatric cohorts, emphasizing the importance of individualized assessment. Digital health tools and artificial intelligence-driven algorithms are being developed to augment CGA, providing dynamic, real-time risk evaluation and treatment monitoring.
International guidelines from the International Society of Geriatric Oncology (SIOG), American Society of Clinical Oncology (ASCO), and European Hematology Association (EHA) endorse routine implementation of CGA in older adults with cancer. These guidelines advocate for pre-therapy assessment of functional, cognitive, and psychosocial domains, with CGA findings directly influencing therapy selection, supportive care planning, and patient counseling. Integration of CGA is associated with improved outcomes, reduced toxicity, and enhanced quality of life.
Comprehensive geriatric assessment is an indispensable component of pre-therapeutic evaluation in older adults with hematologic malignancies. By systematically identifying vulnerabilities, guiding individualized therapy, and informing multidisciplinary care, CGA bridges the gap between chronological age and biological fitness. Emerging evidence and evolving guidelines reinforce its role in optimizing outcomes and minimizing harm. Continued research and integration of novel assessment tools will further enhance the precision and impact of geriatric oncology care.
