Abstract

Maternal vascular function plays a pivotal role in determining both maternal and fetal outcomes during pregnancy. Recent advances underscore the necessity for standardized clinical assessment of vascular health in pregnant women to improve risk stratification and management of hypertensive disorders, preeclampsia, and other vascular complications. This comprehensive review synthesizes current evidence and guideline recommendations for the assessment of maternal vascular function throughout pregnancy, providing clinicians with practical and mechanistic insights to inform patient care.

Introduction

Assessment of maternal vascular function has gained increasing attention due to its fundamental role in pregnancy adaptation and the pathogenesis of adverse outcomes such as preeclampsia, fetal growth restriction, and maternal cardiovascular morbidity. The physiologic changes in maternal vasculature, including endothelial function and arterial compliance, are dynamic and closely related to perinatal health. Rapid progression to severe disease underscores the need for vigilant clinical monitoring.

Diagnosis

Assessment of maternal vascular function incorporates a combination of clinical, biochemical, and imaging modalities. Blood pressure monitoring remains the cornerstone of diagnosis, but advanced tools such as flow-mediated dilation (FMD), pulse wave velocity (PWV), and uterine artery Doppler ultrasonography provide mechanistic insights and risk prediction. Biochemical markers—including placental growth factor (PlGF) and soluble fms-like tyrosine kinase-1 (sFlt-1)—aid in differentiating preeclampsia from other hypertensive disorders. Serial assessments throughout gestation are recommended for high-risk populations.

Treatment & Management

Management strategies are guided by the severity and etiology of vascular dysfunction. For hypertensive disorders, antihypertensive therapy, close monitoring, and timely delivery are key interventions. Aspirin prophylaxis (81-150 mg daily) is recommended for women at high risk for preeclampsia, initiated between 12 and 16 weeks of gestation. Lifestyle modification, including dietary optimization and physical activity, supports vascular health. Multidisciplinary care involving obstetricians, cardiologists, and maternal-fetal medicine specialists enhances outcomes in complex cases.

Recent Advances / Emerging Therapies

Recent years have witnessed the evolution of novel biomarkers and imaging techniques for early detection of maternal vascular dysfunction. Research into the utility of serum angiogenic factors, microRNAs, and proteomic signatures holds promise for personalized risk assessment. Noninvasive vascular imaging, such as 3D Doppler and vascular elastography, is being explored for enhanced diagnostic accuracy. Emerging therapies targeting endothelial dysfunction and oxidative stress are under investigation but remain experimental. Integration of machine learning algorithms with clinical data may further refine risk prediction models in the near future.

Guideline Recommendations

Early initiation of aspirin prophylaxis is a cornerstone for high-risk women. Multidisciplinary care and patient education are emphasized to ensure optimal maternal and fetal outcomes.
