Abstract

Metabolic surgery, primarily bariatric procedures such as gastric bypass and sleeve gastrectomy, has emerged as a cornerstone intervention for severe obesity and related metabolic disorders. Beyond weight loss, accumulating evidence indicates substantial long-term health impacts for women, spanning reproductive, cardiovascular, oncological, and bone health domains. This review synthesizes recent scientific findings on the long-term health outcomes for women post-metabolic surgery, discusses underlying mechanisms, delineates risk factors and clinical features, and provides guideline-based recommendations to inform clinical practice. The article emphasizes the need for vigilant, multidisciplinary follow-up to optimize women’s health after metabolic surgery.

Introduction

Obesity is a pervasive public health challenge, disproportionately affecting women globally and predisposing them to a spectrum of metabolic, cardiovascular, reproductive, and oncological diseases. Metabolic surgery has become an established treatment for severe obesity, with profound effects on glycemic control and metabolic syndrome components. In recent years, attention has shifted toward understanding the extended impact of these surgical interventions on women’s health beyond weight reduction. This review aims to provide a comprehensive, evidence-based overview of the long-term consequences both beneficial and adverse of metabolic surgery in women, targeting clinicians seeking to optimize patient outcomes through informed, guideline-aligned care.

Epidemiology / Disease Burden

Obesity affects over 650 million adults worldwide, with women representing a larger proportion in most regions. The prevalence of severe obesity (BMI ≥40 kg/m²) is rising, particularly among women of reproductive age. Postoperative surveillance is essential to detect long-term complications in women. Diagnostic strategies include periodic assessment of glycemic control, reproductive hormone panels, dual-energy X-ray absorptiometry (DEXA) for bone mineral density, and laboratory screening for iron, vitamin B12, folate, calcium, and vitamin D. Gynecologic evaluation should be considered for menstrual or fertility disturbances. Cancer screening protocols should be adjusted for changes in risk profile following substantial weight loss and hormonal modulation.

Treatment & Management

Multidisciplinary collaboration between bariatric surgeons, endocrinologists, gynecologists, and dietitians is vital. Osteoporosis prevention strategies include adequate calcium and vitamin D intake and weight-bearing exercise.

Recent Advances / Emerging Therapies

Recent advances focus on optimizing surgical techniques for better metabolic outcomes and reduced nutritional risks, including sleeve gastrectomy with duodenal-jejunal bypass and single-anastomosis procedures. Novel pharmacotherapies (GLP-1 agonists, SGLT2 inhibitors) are being integrated with surgery for weight maintenance and metabolic control. Emerging therapies targeting gut microbiota and personalized supplementation regimens are under investigation to further improve long-term women’s health outcomes. Metabolic surgery offers transformative benefits for women with severe obesity, extending beyond weight loss to encompass improved metabolic, reproductive, and oncological outcomes. However, these benefits are counterbalanced by potential risks of nutritional deficiencies, bone disease, and reproductive complications. Optimal long-term health requires vigilant, multidisciplinary follow-up, guideline-based supplementation, and individualized risk stratification. Ongoing research and innovation will continue to refine strategies for maximizing benefits and minimizing risks, ensuring the best possible outcomes for women undergoing metabolic surgery.
