Integrated therapeutic pathway harmonization refers to the systematic alignment and coordination of treatment protocols across disciplines, disease states, and therapeutic modalities. The aim is to reduce unwarranted variability, enhance medication safety, and improve clinical efficacy in complex patient populations. As medicine becomes increasingly personalized, the role of clinical pharmacology in ensuring harmonized, patient-centric care pathways has garnered significant attention. This review synthesizes recent evidence and expert consensus to provide clinicians with a comprehensive understanding of pathway harmonization in clinical pharmacology, addressing the scientific rationale, operational challenges, and practical implications for daily practice.

Recent advances in clinical pharmacology have accelerated the evolution of harmonized therapeutic pathways.
