In severe cases, chronic intestinal pseudo-obstruction may mimic mechanical obstruction, leading to recurrent hospitalizations, nutritional deficits, and significant morbidity. Symptom patterns often fluctuate and may be exacerbated by stress, comorbidities, or changes in medication.
Diagnosis requires a combination of clinical acumen and specialized investigations. Initial evaluation includes detailed history, physical examination, and exclusion of mechanical causes via imaging (abdominal X-ray, CT, or MRI). Advanced diagnostics encompass gastrointestinal transit studies, antroduodenal or colonic manometry, and, in selected cases, full-thickness biopsies for histopathological evaluation. General principles include symptomatic control, correction of nutritional deficiencies, and prevention of complications. Pharmacological therapies encompass prokinetic agents (e.g., prucalopride, lubiprostone), antispasmodics, and, in selected cases, immunosuppressive medications for autoimmune-mediated processes. Non-pharmacological interventions such as dietary modification, fluid management, and physical therapy may confer additional benefit. Severe or refractory cases may require enteral or parenteral nutrition, decompressive procedures, or surgical intervention.
Recent years have witnessed significant advancements in the understanding and management of intestinal neuromuscular disorders. Novel agents targeting specific neuromodulatory pathways, such as selective serotonin receptor agonists and chloride channel activators, have expanded therapeutic options. Stem cell therapy and regenerative approaches aiming to restore ENS and ICC function are under active investigation. Non-invasive neuromodulation techniques, such as sacral nerve stimulation and transcutaneous electrical stimulation, have demonstrated promise in refractory cases. Advances in gut microbiome research offer further avenues for therapeutic intervention, including probiotics and microbiota-directed therapies.
Contemporary clinical guidelines emphasize a stepwise, evidence-based approach to diagnosis and management. The American Gastroenterological Association and related bodies recommend early recognition, exclusion of mechanical obstruction, and use of validated manometric techniques for diagnostic confirmation. First-line therapy typically involves dietary and lifestyle modification, escalating to pharmacological agents as indicated. Multidisciplinary care is advocated for complex cases, with consideration of advanced therapies and surgical options in selected patients. Ongoing patient education and regular follow-up are critical to optimize outcomes and minimize complications.
