Abstract

Effective public health strategies aimed at creating healthy food environments are paramount in the sustainable management of body composition. This review synthesizes recent scientific evidence on the epidemiology, pathophysiology, risk factors, clinical presentation, diagnostic approaches, and evidence-based management of body composition disorders, with a focus on the role of food environments. It also explores recent advances, emerging therapies, and current guideline recommendations, offering clinicians practical and mechanistic insights for translating public health principles into clinical and community practice.

Introduction

Body composition management, encompassing the optimization of fat mass and preservation of lean muscle, has emerged as a central priority in both clinical medicine and public health. Escalating rates of obesity, sarcopenia, and metabolic syndrome are intricately linked to the environments in which individuals make food choices. The concept of the "food environment" encapsulates accessibility, availability, affordability, and marketing of foods, all of which critically shape dietary behaviors and long-term health outcomes. This article reviews the scientific underpinnings of healthy food environments and their pivotal role in sustainable body composition management, catering to healthcare professionals seeking evidence-based, mechanistically sound interventions.

Epidemiology / Disease Burden

Globally, the prevalence of overweight and obesity has reached epidemic proportions, with the World Health Organization estimating that over 1.9 billion adults are overweight and more than 650 million are obese. Laboratory investigations may include fasting glucose, lipid profiles, inflammatory markers, and liver function tests. Emerging modalities, such as MRI and advanced metabolomics, offer granular insights into ectopic fat deposition and metabolic health, although practicality and cost remain considerations for routine clinical use.

Treatment & Management

Management strategies are most effective when they integrate individual- and community-level interventions. Clinically, evidence-based dietary prescriptions emphasize caloric moderation, increased consumption of whole foods, plant-based proteins, dietary fiber, and healthy fats, alongside regular physical activity. Motivational interviewing, behavioral counseling, and structured weight management programs are critical for sustained lifestyle change. Additionally, digital health platforms leveraging real-time dietary tracking, personalized feedback, and telemedicine are increasingly integrated into body composition management paradigms. Guideline Recommendations

International and national guidelines uniformly advocate for the creation of supportive food environments as a cornerstone of obesity and body composition management. The WHO, American Heart Association, and European Society of Endocrinology recommend policies that enhance access to affordable, nutritious foods, restrict marketing of unhealthy products to children, and promote food literacy through education. Clinicians are encouraged to assess social determinants of health, advocate for environmental change, and collaborate with public health authorities to implement multi-level interventions. Regular monitoring, outcome evaluation, and adaptation to local context are emphasized for sustained success.
