Liver cirrhosis, a late stage of progressive hepatic fibrosis, is characterized by the distortion of the normal liver architecture and the development of regenerative nodules. This chronic liver disease is often associated with a wide range of complications that pose significant challenges to healthcare professionals.
Portal hypertension is a common complication of liver cirrhosis, leading to varices and ascites. Management includes the use of non-selective beta-blockers and endoscopic therapies to prevent variceal bleeding. Diuretics and sodium restriction are the mainstays of ascites management.
This neuropsychiatric syndrome is another major complication, characterized by personality changes, intellectual impairment, and a depressed level of consciousness. Treatment typically includes the reduction of ammonia production and absorption using lactulose or rifaximin.
Hepatorenal syndrome, a type of prerenal kidney injury, is a serious complication that requires timely diagnosis and management. Therapeutic strategies include volume expansion, use of vasoconstrictors, and, in severe cases, renal replacement therapy or liver transplantation.
Liver cirrhosis increases the risk of hepatocellular carcinoma. Regular surveillance using ultrasound, with or without alpha-fetoprotein measurement, is recommended. Treatment options include surgical resection, liver transplantation, and locoregional therapies.
Understanding and managing the complex complications of liver cirrhosis require a comprehensive and multidisciplinary approach. By recognizing these complications early and initiating appropriate treatment, healthcare professionals can significantly improve the prognosis and quality of life for patients with this chronic liver disease.
