Pyrexia, commonly referred to as fever, is an elevation in the body's basal temperature and is a common clinical condition encountered in medical practice. It is a physiological response to a variety of conditions, most notably infections, but also malignancies, autoimmune diseases, and certain medications.
Fever is a complex process orchestrated by the hypothalamus in response to pyrogens. Exogenous pyrogens include microbial substances such as lipopolysaccharides, while endogenous pyrogens are typically cytokines produced by the immune system. These pyrogens trigger the synthesis of prostaglandin E2 (PGE2) in the hypothalamus, which resets the body's 'thermostat' to a higher temperature, leading to fever.
An accurate history and physical examination are crucial in the evaluation of a patient with fever. The history should focus on the duration, pattern, and associated symptoms of the fever, while the examination should seek signs of localized infections or systemic diseases. Investigations should be guided by the clinical findings and may include complete blood counts, cultures, and imaging studies.
Management of pyrexia primarily involves treating the underlying cause. Antipyretics such as paracetamol and nonsteroidal anti-inflammatory drugs (NSAIDs) can be used to alleviate discomfort. However, the use of antipyretics should be balanced against the potential benefits of fever in fighting infections. In cases of hyperpyrexia, immediate measures to reduce body temperature, including cooling techniques and antipyretic medications, are essential to prevent serious complications.
Pyrexia is a common clinical phenomenon with a diverse range of causes. A thorough understanding of the physiological mechanisms, careful assessment, and appropriate management strategies are vital for effective patient care. It is also important to recognize that fever is not merely a symptom to be suppressed, but a complex physiological response that may contribute to host defense.
