The evaluation begins with a detailed clinical history and physical examination. This includes understanding the patient's symptoms, their onset, progression, and associated features. A thorough neurological examination can help identify the pattern of nerve involvement, providing clues towards the underlying cause.
Following the clinical assessment, laboratory testing is essential. Basic tests include complete blood count, glucose levels, renal and liver function tests, and Vitamin B12 levels. Depending on the initial findings, more specific tests like serum protein electrophoresis or genetic testing may be warranted.
Electrodiagnostic studies such as nerve conduction studies and electromyography can provide valuable information about the type and extent of nerve damage. These studies help confirm the diagnosis, localize the lesion, and sometimes, suggest the etiology.
Imaging studies, particularly MRI, can be useful in certain cases of peripheral neuropathy, especially when a compressive lesion is suspected. In selected cases, nerve biopsy may be necessary to confirm the diagnosis and guide treatment.
