Gastroesophageal reflux disease (GERD) represents a prevalent and complex disorder, with multifaceted pathophysiology and clinical manifestations. The diagnostic and therapeutic challenges it presents necessitate a comprehensive understanding for effective patient management.
Diagnosis of GERD primarily relies on clinical evaluation, with patients frequently presenting with heartburn and regurgitation. Upper endoscopy is recommended in patients with alarm symptoms or refractory GERD. However, normal endoscopic findings do not exclude GERD, necessitating further investigation. Ambulatory pH monitoring and esophageal manometry are crucial diagnostic tools, providing objective evidence of acid reflux and esophageal motor function, respectively.
Proton pump inhibitors (PPIs) remain the cornerstone of GERD treatment, effectively relieving symptoms and healing esophagitis. However, a significant subset of patients exhibit PPI-refractory GERD, necessitating alternative therapeutic strategies. Alginate-based therapies, prokinetic drugs, and transient lower esophageal sphincter relaxation (TLESR) reducers show promise in these cases.
Laparoscopic antireflux surgery (LARS) is indicated for patients with well-documented GERD who are either PPI-refractory or prefer surgery to lifelong medication. Endoscopic treatments, such as radiofrequency ablation (Stretta) and transoral incisionless fundoplication (TIF), are emerging as viable alternatives to LARS, particularly for patients with uncomplicated GERD.
GERD's complexity necessitates continued research into its pathophysiology, diagnostic techniques, and treatment modalities. Novel approaches, such as impedance-pH monitoring and personalized medicine, hold promise for improving patient outcomes.
GERD is a complex disorder requiring a comprehensive approach to diagnosis and treatment. While PPIs remain the mainstay of treatment, a significant subset of patients require alternative therapies, including surgical and endoscopic interventions. Continued research and advancements in diagnostic and therapeutic modalities promise to improve patient outcomes.
