Antimicrobial resistance (AMR) is a significant global health concern, compromising the effective treatment of infectious diseases. In hospitals, the judicious use of antimicrobials, known as antimicrobial stewardship (AMS), plays a pivotal role in mitigating AMR and enhancing patient care.
AMS is a coordinated program that promotes the appropriate use of antimicrobials to improve patient outcomes, reduce microbial resistance, and decrease the spread of infections caused by multidrug-resistant organisms. It involves the selection of an optimal antimicrobial drug regimen, including dosing, duration of therapy, and route of administration.
AMS programs can lead to improved patient outcomes, reduced healthcare costs, and minimized adverse events associated with antimicrobial use. By preventing the overuse and misuse of antimicrobials, these programs can significantly reduce the prevalence of AMR, thereby preserving the effectiveness of existing antimicrobial drugs.
Successful implementation of AMS programs requires a multidisciplinary approach involving infectious disease physicians, pharmacists, clinical microbiologists, and infection control practitioners. Key strategies include preauthorization of broad-spectrum antimicrobials, prospective audit with intervention and feedback, education, guidelines and clinical pathways, and antimicrobial cycling and mixing.
Despite the clear benefits, implementation of AMS programs can face several barriers, including lack of resources, resistance from healthcare professionals, and regulatory issues. Future directions should focus on overcoming these challenges, enhancing collaboration and communication among healthcare professionals, and integrating AMS programs into the hospital's quality improvement initiatives.
In conclusion, AMS plays a crucial role in optimizing patient care and combating AMR in hospitals. It is imperative for healthcare professionals to understand the concept of AMS and actively participate in its implementation to ensure the judicious use of antimicrobials and safeguard public health.
