Peptic ulcer disease (PUD) affects a significant portion of the global population, presenting a substantial burden to healthcare systems. Despite its prevalence, the management of PUD has evolved significantly over the past few decades, owing to advancements in diagnostic tools, pharmacological interventions, and endoscopic techniques.
Modern diagnostic tools have revolutionized the detection of PUD. The advent of high-definition endoscopy and contrast-enhanced endoscopic ultrasound has enhanced the visualization of mucosal patterns and vascular structures, leading to more accurate diagnosis. Additionally, non-invasive tests for Helicobacter pylori, the most common cause of PUD, have improved detection rates and guided targeted therapy.
Pharmacological management of PUD has witnessed significant advancements. Proton pump inhibitors (PPIs) remain the cornerstone of therapy, providing rapid symptom relief and healing of ulcers. The introduction of vonoprazan, a novel potassium-competitive acid blocker, has demonstrated superior efficacy in healing and preventing ulcer recurrence. Furthermore, advancements in H. pylori eradication regimens, including quadruple and concomitant therapy, have improved eradication rates and reduced antibiotic resistance.
Endoscopic techniques have evolved to manage complicated PUD. Endoscopic submucosal dissection and endoscopic mucosal resection have emerged as effective techniques for managing gastric ulcers with low-grade dysplasia. For bleeding ulcers, endoscopic hemostasis techniques, such as endoscopic clipping and argon plasma coagulation, have improved outcomes significantly.
Advancements in the management of PUD have led to improved patient outcomes, with more accurate diagnosis, effective pharmacological interventions, and innovative endoscopic techniques. Ongoing research and development promise further improvements in PUD management, necessitating healthcare professionals to stay abreast of these advancements to provide optimal patient care.
