PD is characterized by the progressive loss of dopaminergic neurons in the substantia nigra pars compacta, leading to striatal dopamine deficiency. The accumulation of alpha-synuclein in Lewy bodies is a key pathological feature. The exact cause of neuronal death is unclear, but genetic and environmental factors have been implicated.
PD typically presents with motor symptoms such as bradykinesia, rigidity, and resting tremor. Non-motor symptoms can include autonomic dysfunction, sleep disturbances, and cognitive impairment. The disease's progression is variable, with some patients remaining functionally independent for many years, while others experience rapid decline.
Diagnosis is primarily clinical, based on the presence of characteristic motor symptoms and response to dopaminergic therapy. Neuroimaging can support the diagnosis but is not routinely used due to its limited sensitivity and specificity.
Treatment focuses on managing symptoms and improving quality of life. Pharmacological therapy, including levodopa and dopamine agonists, forms the cornerstone of management. Non-pharmacological interventions, such as physiotherapy and occupational therapy, also play a crucial role. Deep brain stimulation may be considered in patients with refractory symptoms.
In conclusion, PD is a complex neurodegenerative disorder with diverse clinical manifestations and a variable course. A deep understanding of its pathophysiology, clinical presentation, diagnosis, and treatment options is essential for healthcare professionals to provide optimal patient care.
