Enteric fever, primarily caused by Salmonella Typhi and Paratyphi, remains a significant global health issue. Despite advancements in diagnostic tools and therapeutic strategies, challenges persist in clinical practice. This article aims to provide an updated approach to the diagnosis and management of enteric fever.
Accurate diagnosis of enteric fever is critical. Traditional methods, such as blood culture, remain the gold standard but suffer from sensitivity issues. Recent advances in molecular diagnostics, including PCR-based tests, offer improved sensitivity and specificity. Serological tests like Widal and Typhidot are commonly used in resource-limited settings, but their reliability is questionable due to cross-reactivity and variability in antibody response. Therefore, integrating these methods with clinical suspicion and epidemiological data is crucial.
Antimicrobial therapy is the cornerstone of enteric fever management. Resistance to first-line drugs, such as chloramphenicol, ampicillin, and trimethoprim-sulfamethoxazole, has led to the use of fluoroquinolones. However, the emergence of fluoroquinolone-resistant strains necessitates alternative therapies. Third-generation cephalosporins and azithromycin have shown promising results. The choice of antimicrobial should be guided by local susceptibility patterns and patient characteristics.
Supportive care, including hydration and symptom management, is crucial. Vaccination remains the most effective preventive measure. Currently, two vaccines (Ty21a and Vi polysaccharide) are available, but their protective efficacy varies. Improved sanitation and safe food practices are also essential in preventing enteric fever.
Enteric fever continues to pose significant challenges due to diagnostic limitations and antimicrobial resistance. An integrated approach, encompassing accurate diagnosis, appropriate antimicrobial therapy, supportive care, and preventive measures, is crucial. Further research is needed to develop more effective diagnostic tools, therapeutic strategies, and vaccines to combat this persistent global health threat.
