Cardiovascular risk assessment in diabetic patients should include a thorough medical history, physical examination, and appropriate diagnostic testing. Key factors to consider include age, gender, ethnicity, family history of CVD, smoking status, hypertension, dyslipidemia, and presence of microvascular complications. Further, the use of risk calculators such as the Framingham Risk Score or the American College of Cardiology/American Heart Association ASCVD Risk Estimator can provide quantitative estimates of CVD risk.
Management of cardiovascular risk in diabetic patients should focus on lifestyle modifications, pharmacotherapy, and regular follow-up. Lifestyle modifications include dietary changes, regular physical activity, smoking cessation, and weight management. Pharmacotherapy options include glucose-lowering agents, antihypertensive medications, lipid-lowering drugs, and antiplatelet therapy. Importantly, choice of pharmacotherapy should consider patient preferences, potential side effects, and drug interactions.
Recent studies suggest that certain glucose-lowering agents may offer cardiovascular benefits beyond their glycemic effects. Sodium-glucose cotransporter 2 (SGLT2) inhibitors and glucagon-like peptide-1 (GLP-1) receptor agonists have shown to reduce major adverse cardiovascular events in high-risk diabetic patients. Healthcare professionals should stay up-to-date with the latest evidence to optimize patient outcomes.
