Unexplained weight loss (UWL), defined as an unintentional loss of more than 5% of body weight within a six-month period, can be a harbinger of serious underlying diseases. The diagnostic approach to UWL can be challenging due to the broad differential diagnosis. This article aims to provide a comprehensive approach to diagnose UWL.
Initial evaluation should focus on a detailed history and physical examination. It's imperative to rule out intentional weight loss, medication side effects, and psychiatric illnesses. A thorough history should include questions about changes in appetite, food intake, and dietary habits. Ask about symptoms of malignancy, endocrine disorders, gastrointestinal diseases, and chronic infections.
Basic laboratory tests, including complete blood count, comprehensive metabolic panel, thyroid function tests, and urinalysis, should be conducted. Additional tests like HIV testing, imaging studies, or endoscopy may be required based on the clinical suspicion.
Geriatric patients may present with weight loss due to physiological changes, decreased appetite, or social factors. In these patients, a diagnosis of exclusion, such as 'frailty' or 'failure to thrive', may be considered after a thorough evaluation.
Management should be directed towards the underlying cause. In cases where no cause is identified, symptomatic management with nutritional support and appetite stimulants may be considered. Regular follow-ups are crucial to monitor the patient's weight and response to treatment.
Diagnosing unexplained weight loss requires a methodical and comprehensive approach. A detailed history, physical examination, and judicious use of diagnostic tests form the cornerstone of evaluation. Timely identification and management of the underlying cause can significantly improve patient outcomes.
