Tuberculosis (TB) remains a significant global health challenge, necessitating the continuous evolution of diagnostic strategies. Recent advancements have broadened the scope of TB detection, improving both sensitivity and specificity. This article reviews the latest developments and approaches in TB detection.
Microbiological testing remains the gold standard in TB diagnosis. Modern techniques include automated liquid culture systems, which have improved sensitivity and shortened turnaround times. Additionally, molecular line probe assays provide rapid, simultaneous detection of TB and resistance to rifampicin and isoniazid.
GeneXpert MTB/RIF, a real-time PCR assay, has revolutionized TB detection. It offers simultaneous detection of TB and rifampicin resistance within two hours. Moreover, the advent of GeneXpert Ultra enhances sensitivity, particularly in paucibacillary forms of TB such as pediatric and extrapulmonary TB.
Interferon-gamma release assays (IGRAs) and tuberculin skin tests (TSTs) are immunological tests used in TB detection. Although not definitive diagnostic tools, they are useful in identifying latent TB infection. IGRAs, with their higher specificity, are particularly valuable in BCG-vaccinated populations.
Imaging continues to play an integral role in TB detection. The advent of digital radiography and computer-aided detection systems has improved the sensitivity and specificity of radiological diagnosis. Newer imaging modalities, such as positron emission tomography-computed tomography (PET-CT), offer valuable insights in complex cases.
In conclusion, advancements in TB detection encompass a range of techniques, including microbiological, molecular, immunological, and imaging-based methods. These developments aim to improve diagnostic accuracy, reduce turnaround times, and facilitate early initiation of treatment. However, challenges persist, emphasizing the need for ongoing research and innovation in TB detection methods.
