Unexplained weight loss (UWL) is a common clinical conundrum that often signifies an underlying serious illness. It is defined as the unintentional loss of more than 5% of body weight within a six to twelve-month period. This article aims to provide a comprehensive approach to diagnosing UWL in patients.
History taking and physical examination should be the first step in evaluating UWL. The physician should gather information about the duration, rate, associated symptoms, dietary intake, lifestyle changes, and psychosocial history. A comprehensive physical examination can often provide clues to the underlying cause.
After the initial evaluation, a stepwise diagnostic approach is recommended. Basic laboratory tests including a complete blood count, metabolic panel, liver function tests, thyroid function tests, and urinalysis should be performed. Depending on the findings, further targeted investigations may be required.
In cases where history, physical examination, and basic laboratory tests do not provide a diagnosis, imaging studies may be necessary. Abdominal ultrasound, CT scan, or MRI can be used to detect malignancies, chronic infections, or endocrine disorders.
If the cause of UWL remains elusive after these steps, referral to a specialist may be necessary. Depending on the suspected underlying cause, this could be a gastroenterologist, endocrinologist, psychiatrist, or oncologist.
Unexplained weight loss is a challenging clinical scenario that requires a systematic and comprehensive approach. A thorough history and physical examination, followed by targeted investigations, can often lead to a diagnosis. In cases where the cause remains elusive, referral to a specialist may be necessary. By following this approach, physicians can ensure that serious underlying conditions are not missed, and patients can receive appropriate treatment in a timely manner.
