Acute chest pain (ACP) is a common symptom that presents in various clinical settings, often posing a diagnostic challenge for clinicians due to its broad differential diagnoses. This article aims to provide a comprehensive approach to evaluating and managing ACP, aiding in the prompt identification and treatment of potential life-threatening conditions.
Initial evaluation should focus on obtaining a detailed history and performing a thorough physical examination. Key elements of the history include the nature, location, duration, radiation, and associated symptoms of the pain. Risk factors for coronary artery disease should also be assessed. Physical examination findings can provide clues to specific causes of chest pain and guide further investigations.
Electrocardiogram (ECG) and cardiac biomarkers are the cornerstone of initial diagnostic testing. An ECG should be obtained immediately in all patients presenting with ACP. Cardiac biomarkers, such as troponin, should be measured on presentation and repeated as clinically indicated. Other diagnostic tests, including chest radiography or computed tomography, may be useful depending on the clinical context.
Management strategies depend on the underlying cause of the ACP. In patients with suspected acute coronary syndrome, prompt revascularization is critical. For other causes of ACP, such as pulmonary embolism or aortic dissection, specific treatment protocols should be followed. It is also important to consider non-cardiac causes of chest pain, such as gastrointestinal or musculoskeletal conditions, which may require different management approaches.
ACP is a complex clinical symptom that requires a systematic and comprehensive approach for evaluation and management. By understanding the different potential causes and appropriate diagnostic tests, clinicians can effectively diagnose and manage patients presenting with ACP, potentially reducing morbidity and mortality.
