Unexplained weight loss (UWL), defined as a loss of more than 5% of body weight over a period of 6-12 months without a clear cause, is a common yet challenging presentation in clinical practice. It is often associated with significant morbidity and mortality, necessitating a comprehensive and systematic approach to its evaluation.
UWL can be a manifestation of various underlying conditions ranging from malignancies, gastrointestinal disorders, endocrine diseases, to psychiatric conditions. It can also be a side effect of certain medications or a result of social factors such as poverty and isolation. Understanding the broad etiology is crucial to formulating a structured diagnostic approach.
A thorough history and physical examination remain the cornerstone of the initial evaluation. Key aspects include dietary history, medication review, psychosocial evaluation, and a detailed examination for signs of systemic disease. The presence of 'red flag' symptoms such as pain, bleeding, or dysphagia warrant immediate further investigation.
Following the initial assessment, a stepwise diagnostic approach is recommended. Basic investigations including complete blood count, renal and liver function tests, thyroid function tests, and chest X-ray should be performed. If these are inconclusive, targeted investigations based on the clinical suspicion should be considered. These could include endoscopy, imaging studies, or specific serological tests.
Management of UWL should be directed towards treating the underlying cause, if identified. In cases where no cause is found, symptomatic management with nutritional support and regular monitoring is essential. Early involvement of a multidisciplinary team can improve outcomes in these patients.
Evaluating unexplained weight loss in clinical practice requires a comprehensive, systematic approach encompassing a thorough clinical assessment, a stepwise diagnostic strategy, and appropriate management. While this can be challenging, it is crucial in order to identify potential underlying serious conditions and to ensure optimal patient care.
