Fatigue, a common clinical complaint, often presents a diagnostic challenge due to its multifactorial nature. This article aims to provide a comprehensive clinical approach to understanding and managing fatigue.
Fatigue can be categorized into physical and mental fatigue. Physical fatigue is characterized by a decrease in physical performance, while mental fatigue is a subjective feeling of tiredness, lack of energy, or exhaustion. It's important to note that fatigue is not synonymous with sleepiness, although the two can coexist.
When approaching a patient with fatigue, a detailed history and physical examination are crucial. The history should include onset, duration, severity, and any associated symptoms. Lifestyle factors such as sleep hygiene, diet, exercise, and stress levels should also be considered. Physical examination should focus on signs of systemic diseases that could cause fatigue.
Investigations should be guided by the clinical suspicion. Basic investigations may include complete blood count, thyroid function tests, glucose levels, renal and liver function tests. More specific tests, such as autoimmune markers or sleep studies, may be required based on the clinical picture.
Treatment of fatigue is primarily directed at the underlying cause. Symptomatic treatment may include lifestyle modifications, cognitive-behavioral therapy, and pharmacological interventions. Regular follow-ups are necessary to assess the response to treatment and adjust the management plan accordingly.
Fatigue is a complex symptom with a broad differential diagnosis. A comprehensive clinical approach, including detailed history taking, thorough physical examination, and targeted investigations, is necessary to accurately diagnose and effectively manage fatigue.
