Vertigo, commonly mistaken as a disease, is a symptom indicating a disorder in the vestibular system. It is characterized by a sensation of spinning or movement when the body is stationary. Differentiating vertigo from dizziness, which is a more generalized term, is crucial in clinical practice.
The diagnosis of vertigo involves a detailed patient history, physical examination, and targeted investigations. The onset, duration, and accompanying symptoms can provide valuable clues to the underlying cause. Dix-Hallpike and head impulse tests are essential bedside maneuvers. Neuroimaging and vestibular testing may be warranted in certain cases.
Benign paroxysmal positional vertigo (BPPV), Meniere's disease, and vestibular migraine are among the common causes of peripheral vertigo. Central causes include cerebrovascular disease and multiple sclerosis. A comprehensive understanding of these conditions is vital for accurate diagnosis and treatment.
Treatment of vertigo is primarily aimed at managing the underlying cause. For BPPV, canalith repositioning maneuvers are the first-line treatment. Meniere's disease may require a combination of dietary modification, medication, and in some cases, surgery. Vestibular rehabilitation is often beneficial across a range of vertigo etiologies.
Vertigo, while common, remains a complex clinical entity requiring a systematic approach for effective management. By understanding its underlying mechanisms, employing a thorough diagnostic approach, and applying appropriate treatment strategies, healthcare professionals can significantly improve patient care and outcomes in vertigo management.
