Acute chest pain (ACP) is a common yet challenging symptom encountered in the medical field. It can be a manifestation of numerous conditions, ranging from benign to life-threatening. Therefore, a systematic and comprehensive approach is crucial for its evaluation and management.
The etiology of ACP is multifaceted. Cardiovascular causes, such as acute coronary syndrome, myocarditis, and aortic dissection, are among the most severe. However, other systems can also contribute to ACP, including pulmonary conditions like pneumonia and pulmonary embolism, gastrointestinal issues such as gastroesophageal reflux disease and peptic ulcer disease, and musculoskeletal causes like costochondritis.
A thorough history and physical examination are paramount in the evaluation of ACP. Key factors include the character, location, duration, and radiation of the pain, associated symptoms, and risk factors for cardiovascular disease. Diagnostic tests such as electrocardiogram, chest radiograph, and cardiac biomarkers can provide valuable information. Advanced imaging, like CT angiography, may be necessary in certain cases.
Management of ACP is guided by the underlying cause. Immediate intervention is required for life-threatening conditions, such as revascularization in acute coronary syndrome. For less severe causes, symptomatic treatment and addressing the root cause, such as acid suppression in GERD, are usually effective. Patient education and reassurance also play an essential role in management.
ACP is a complex symptom with a broad differential diagnosis. A comprehensive approach, incorporating a thorough history, physical examination, and appropriate diagnostic testing, is essential in guiding management. By understanding the diverse etiologies of ACP and tailoring the treatment strategy to the underlying cause, healthcare professionals can significantly impact patient outcomes.
