The rise of antimicrobial resistance (AMR) poses a significant threat to global health. In hospital settings, the judicious use of antimicrobials is crucial to combat this issue. Herein, we discuss strategies for effective antimicrobial stewardship (AMS) and how they contribute towards improved patient care.
AMS is an organized approach to optimize the use of antimicrobials. It aims to improve patient outcomes, reduce AMR, and decrease unnecessary costs associated with overuse and misuse of antimicrobials. AMS programs involve a multidisciplinary team of clinicians, pharmacists, microbiologists, and infection control professionals.
1. Prospective Audit and Feedback: Regular review of antimicrobial prescriptions and feedback to the prescriber can significantly improve prescribing practices.
2. Formulary Restriction and Preauthorization: Restricting the use of certain antimicrobials can limit their overuse and misuse.
3. Education and Training: Regular training of healthcare professionals on the principles of AMS can enhance their competency and adherence to guidelines.
4. Antimicrobial Cycling and De-escalation: Regular rotation of antimicrobials and de-escalation from broad to narrow-spectrum agents can help prevent the emergence of resistance.
5. Use of Rapid Diagnostic Tests: These tests can help in early identification of pathogens and appropriate antimicrobial selection.
Effective AMS strategies lead to better patient outcomes by reducing the incidence of drug-resistant infections and adverse drug events. They also ensure that effective antimicrobial agents remain available for future use. Patient safety is enhanced, and hospital stay and associated costs are minimized.
Implementing robust AMS strategies in hospital settings is a critical step towards better patient care. It requires a collaborative effort from all healthcare professionals to ensure the judicious use of antimicrobials and to combat the growing threat of AMR.
